Try, if at all possible, to avoid “don’t know” responses. If you absolutely cannot make a rating based on lack of knowledge, put “DK” in the appropriate box. In filling out this form, use the following definitions:

Reliable means that you can depend on someone to do something they say they will do.

Articulate means that the person orally provided information or discussed issues in a clear, meaningful way.

Active means that the person was involved in the discussion and seemed involved in helping the group achieve its goals.

Prepared means that the person came to class ready to work with learning issues in hand.

Thorough means the person’s reports and discussion contained lots of useful information that resolved the learning issue "completely".

Respectful means that the person appeared interested in what other people had to say and showed concern for other points of view.
