Home Health Feasibility Study

Earlier this week, CFO Bob Rosenberger and ARMC VP Nick Alexander met with caregivers at Adena Home Health and Hospice to discuss the beginning of a feasibility study of our Home Health/Hospice services, as well as those of National Church Residences. Because we are both local, not-for-profit organizations that conduct the same or similar services to patients throughout the region, this study is to:

·  Identify the strengths of each organization and its services;

·  Assess the current and future needs of the community;

·  Determine how Adena and National Church Residences can better serve our patients and their families; and

·  Gather information to help both organizations assess and consider a variety of future options for management and the delivery of home care services in our region

In the best interest of the patients being served, it is the intent of both organizations to use the information collected in the study to determine where we may be able to work together to effectively meet the changing health care needs of the growing number of patients who receive care in their homes. Similar to other feasibility studies we conduct for various projects, this is currently simply a study of services and need; and to assess how we can serve our patients and communities better. Once the results of the study are complete, Adena and National Church Residences leaders will review the recommendations and determine next steps.

State Medical Board of Ohio Releases Lead Testing Education Module

As part of Ohio’s effort to reduce pediatric exposure to lead, the State Medical Board of Ohio and Ohio Department of Health are jointly issuing a new video outlining blood lead testing guidelines for children. Ohio law requires primary health care providers administer blood lead tests to at-risk children.This training will provide information about risk factors, recommendations for screening and medical management of lead poisoning. The module is not mandatory, but is strongly encouraged.

Below is a link to an online video that will guide you through the Blood Lead Testing Requirements. These guidelines will help keep Ohio’s children lead-safe.

Population Health

In March 2016, Population Health initiated a pilot with National Church Residences to assist with increasing quality and decreasing costs in our COPD patient population. We are happy to announce that effective August 1st; NCR has enrolled 100 patients into their service coordination program.

The service coordinator assessed each patient for social determinants related to financial barriers, transportation, medication and health care needs. As a result: 14 patients were referred to home care, 4 patients to hospice and multiple other referrals to community resources which enabled the patients to remain in their home. Partnering with NCR and other community resources enables Population Health to assist patients throughout the care continuum therefore increasing quality, patient experience and decreasing costs.

Inpatient Clinical Documentation Integrity (CDI)Reminder

A secondary diagnosis is a condition that coexists at the time of admission, or develops subsequently. The secondary diagnoses support the severity of illness (SOI), showing that your patient is as sick on paper - as they are in the bed. Secondary diagnoses are defined as those conditions that consume one of the following hospital resources:

1. Clinical evaluation

2. Therapeutic treatment

3. Further evaluation by diagnostic studies, procedures or consultations

4. Extended hospital length of stay (LOS)

5. Increase nursing care and/or monitoring.

Example: If you are monitoring blood pressure and the patient has an order to continue their routine home medication for this condition, Hypertension would qualify as a secondary diagnosis.

Adena Pain Management Hosts State Visit

Two state-level representatives recently visited Adena Bone and Joint for a firsthand account, and frontline information about pain management. Dr. Mark Malinowski and Dr. Andrew Porter offered the representatives real-life insight into the hurdles and challenges current legislation creates for providers and patients when it comes to treatment for pain. The issues really hit home when the group visited the Adena Spine Center to meet with three patients. The visitors heard about the patients’ challenges, and the limitations they face due to insurance formularies. The visit concluded with a behind-the-scenes tour of Adena’s operating rooms and insight on the Health System’s surgical and anesthesiology procedures.
