- Action Items from Last Call:

·  [X] David Shields/Duke Clinical Informatics: flesh out big picture/direction proposal discussed in today’s call further with examples/specifics, to discuss in 2 weeks

·  [X] Ken: ask Galen Mulrooney if he can present on FHIM at next call

- Meeting Notes/Discussion Items:

-  Federal Health Information Model (FHIM) demo by Galen Mulrooney, lead modeler for FHIM

o  Background on FHIM

§  Common model across U.S. health agencies, including VHA, HHS, CMS, DOD, IHS, etc.

§  Harmonizing with RIM-based models, BRIDG, SPL, VHA model, etc.

§  Implementability considered in modeling process

§  In terms of abstraction, lies somewhere at following level:

·  [more abstract] RIM – vMR – FHIM – RMIM [more detailed]

o  Attributes

§  Using stereotypes to identify what corresponding RIM classes are

§  Data types:

·  In previous attempts, problems occurred when model was too close to V3 in terms of data types, due to non-compatibility with other data types (e.g., from V2 messages). Thus, moved to use of more “standard” data types.

·  ~40 data types based on HL7 V3 data types

·  Id and Code are abstract data types, with concrete implementations (e.g., can use HL7 v2 identifier)

§  Discussed white paper by Galen on model transformations [available here]

§  Planning to validate via mapping with functional models (e.g., EHR functional model)

§  Using Open Health Tools modeling tool

o  Current status

§  Various agencies contributed models, working between domains

§  Fully harmonized domains

·  Demographics

·  Security and privacy

§  Domains with almost finished harmonization

·  Eligibility

·  Behavioral health

§  Domains serving as next focus for harmonization

·  Pharmacy

·  Labs

o  Discussion

§  Keith Boone:

·  FHIM appears very intricate and complex; can a more abstract model be used for the vMR?

o  Consensus: yes

§  Could we use the FHIM style for the vMR work?

·  Consensus: yes

§  How could we use the FHIM logistically?

·  Galen can periodically provide zipped up model

·  Modeling is Eclipse-based, and models can be opened up in any Eclipse modeling tools (can be exported into Enterprise Architect, but some model elements are not appropriately handled)

§  Next steps

·  Working session during next week’s call to explore expressing vMR DAM using FHIM style and data types

- Action Items:

[ ] Galen: provide FHIM as zipped up model to Ken, for distribution to CDS list

[ ] Ken: provide vMR UML DAM as EA file to Galen

[ ] Call in 1 week: have working session led by Galen to explore expressing vMR DAM using FHIM style and data types
