April 29, 2016Deadline to sign up for in-school physicals @ the HS and MAMS
We will not accept any packets for in-school physicals after this date, but you will still be able to submit physicals which are completed by your personal doctor until June 15th @ the High School and Middle School
June 1, 2016Deadline to hand in physical paperwork to Lloyd Road Nurse for all 5th Graders going to MAMS in the Fall, 2016
June 15, 2016Deadline for ALL sports paperwork to be received at the High School or Middle school in order to participate in any Summer Camps and Clinics…..this includes Marching Band and Color Guard
August 3, 2016Deadline for ALL Fall Sports and Intramurals

February 22, 2017Deadline for All Spring Sports and Intramurals
Link will open January 20, 2017 and close on February 22, 2017
March 13, 2017Link opens to sign up for in-school physicals at the High School and Middle School
April 28, 2017Deadline to apply for In-School Physical for 2017-2018
We will not accept any packets for in-school physicals after this date, but you will still be able to submit physicals completed by your personal doctor until June 15, 2017.
NOTE: All physical packets and permission forms should be submitted to your child’s school nurse. Please do not give it to the front office, a teacher or counselor.
IMPORTANT information for all High School and Middle School students and parents:
Please know that you only need to submit one physical packet per school year. BUT you must submit a Health History Update Form on line through Google Docs to move from one season to another. If you try to click on the link for the physical packet it will tell you that you have already completed this paperwork; however your child will not be signed up for their next season sport without completion of a Health History Update Form . You MUST submit a Health History Update form for a Winter or Spring Sport by the deadline date indicated above in order for your child to move from one season to another. Revised 5/18/16
