Title:Human Services Professional

Description:

This is a full time, exempt position. HSP’s are responsible for ensuring and adequately documenting that the educational, medical, emotional, and social needs of each child assigned to their case load are met in compliance with all applicable laws, rules, regulations and standards. Collaboration with internal staff and other agencies/organizations is critical in order to provide the best possible outcome for each resident in care.

Principle Duties and Responsibilities:

  1. Manage the individual residential care cases assigned on caseload.
  2. Maintain all resident files for residents on caseload including all required documentation and paperwork per state requirements and maintain files in an ‘audit ready state’ at all times. Assist with peer reviews of resident files.
  3. Serve to aid in the admission decisions for potential new residents, reviewing applications for entrance into the Hephzibah direct care program.
  4. Market the services offered by Hephzibah by developing and maintaining a positive and growing relationship with DFCS case workers and the immediate and extended families of current residents.
  5. Be creative and innovative in suggesting new and better ways to serve residents and aid in the development of new programs and networks of service.
  6. Serve on the Treatment Team in a collaborative manner.
  7. Be on call, when required, for assistance to residents and staff, and maintain an attitude of “do whatever it takes” to meet the needs of residents.
  8. Arrange and provide transportation for residents to and from court hearings, medical, dental and professional counseling appointments - working in concert with direct care staff.
  9. Hold a minimum of two face-to-face meetings a month with each resident on caseload and document needs, concerns, progress, and lack of progress.
  10. Support direct care staff in addressing disciplinary issues and concerns, assisting with consequences and necessary behavior contracts.
  11. Maintain access and integration of state mandated electronic resident databases as required by DHS (GA Score and GA Shine).
  12. Provide advocacy for the residents in court, in school and elsewhere.

Education:

Must have either:

  • A master’s degree in social work, psychology, childhood education or a related field and 1 year of paid work experience in the field of social services or human service delivery

Or

  • A bachelor’s degree in social work, psychology, childhood education or a related field and at least 2 years of paid work experiencein the field of social services or human servicedelivery.

Required Qualifications:

  1. Must agree with and demonstrate a commitment to the mission of Hephzibah.
  2. Must demonstrate a keen interest in working with troubled at-risk foster youth.
  3. Must be able to demonstrate an expansive, applicable competency in the use of all Microsoft Office programs (Word, PowerPoint, Excel, and Outlook).
  4. Must have experience with and be competent in the extensive use of online databases (specificallyGA Score and GA Shines. Prefer someone with experience using these databases)
  5. Must be able to multi-task efficiently.

Physical Working Conditions:

Primarily office related. Occasional travel will be necessary when bringing residents to court or appointments.
