The following is a final report form for the Heart and Stroke Foundation of Saskatchewan (HSFS) Grant-in-Aid (GIA) award. Its purpose is to capture your research achievements over the three year period of your GIA funding. The report will enable the Foundation to fulfill its accountability to donors; leverage fund development by highlighting research successes; and evaluate the success of the GIA program.
