1.POST DESCRIPTION

Introduction

The integrated Sexual Health Service, comprising Contraception/Reproductive Health and Genitourinary Medicine (GUM), is hosted within the Fife Wide Division of the Health and Social care partnership. We deliver and integrated model of sexual and reproductive health care and will therefore require that all medical and nursing staff working within the speciality are appropriately trained in both disciplines.

The Sexual Health Service offers over 30 clinics per week at various locations in Fife including Kirkcaldy, Dunfermline, St. Andrews, Glenrothes, Cupar, Buckhaven, and Cowdenbeath. The majority of clinics are mixed sexual health clinics incorporating both, contraception/reproductive health and genitourinary medicine (GUM).. There are in excess of 20,000 attendances per annum.

Fife has implemented the National Sexual Health IT system (NaSH) . Appropriate training in the use of the NaSH system will be provided for the successful applicant

Services provided:

  • Whole range of contraceptive methods.
  • Testing and treatment for Sexually Transmitted Infections,
  • Dedicated blood borne virus team including pharmacist, psychologists and dietitian
  • Partner Notification, counselling, health-promotion by health advisers.
  • Advice on medical gynaecology & menopause.
  • Sterilisationcounselling and referral.
  • Advice and treatment for other genital tract problems e.g. basic genital dermatological problems.
  • Hepatitis and HPV vaccination for high risk individuals
  • PREP and PEP services
  • There is an active involvement in teaching and training towards the DFSRH and Letters of Competence for intrauterine techniques and subdermal implants (LoC IUT and LoC SDI).
  • The Sexual Health Service has a good working relationship with other services – Microbiology, Obstetrics and Gynaecology, GPs, Public Health and Health Promotion – and both Consultant and nursing staff give talks and presentations to various groups (medical and non-medical, including schools).
  • Sexual Health also works in tandem with Fife Police Forensic Medical Examiners office and Fife Child protection/Community Paediatrics team managing alleged victims of sexual assault/abuse.

Sexual Health Fife Staffing

  • One part time consultant in Reproductive Health.
  • One full time consultant in GUM and HIV care.
  • Six doctors at specialty doctor grade level (~2 WTE), covering clinic sessions. All doctors have undertaken, or are in the process of undertaking, training in both contraception and GUM.
  • In addition there are specialist nurses at various bands, health advisors and clinical support workers. Nursing staff within both services have undertaken, or are in the process of undertaking, dual training in sexual health. Several nurses have completed the nurse-prescribing course, or intend to do so in the future.

2DUTIES AND RESPONSIBILITIES

General Clinical Duties:

Sexual Healthis an exciting speciality that has undergone considerable changes and is at the cutting edge of optimal patient communication and high quality standardised care. The successful candidate will be expected to have an up-to-date Diploma or Membership of the ‘Faculty of Sexual & Reproductive Health – FSRH’ (formerly Faculty of Family Planning & Reproductive Health – FFPRH). Previous experience in Genito-urinary medicine would be ideal but is not essential for the right candidate as training in the relevant areas will be provided. Ability to work as part of a multidisciplinary team is essential.

The post holder will:

  1. Be directly involved management of patients, including young people under age 16 years, providing contraception & sexual health care in out-patient clinic settings with open access and booked appointments.
  2. Work within the national clinical guidelines and local protocols that have already been developed within the department and assist in continually reviewing the existing guidelines/protocols and the development of new ones.
  3. Provide comprehensive advice and assessment for reversible short and long-acting contraceptive methods, including emergency contraception.
  4. Prescribe and issue oral, transdermal and injectable hormonal contraceptives.
  5. Insert and remove subdermal contraceptive implants (Implanon).
  6. Insert and remove intrauterine contraceptive devices (copper-IUD) and the intrauterine system Mirena IUS).
  7. Provide cervical cytology screening following local Screening Guidelines.
  8. Counsel women with an unwanted pregnancy and refer to the Termination of Pregnancy Clinic.
  9. Counsel and refer for male and female sterilisation.
  10. Undertake sexual risk assessment, carry out appropriate investigations and treat various STIs in all patients.
  11. Discuss and offer appropriate advice on safer sex issues to all patients.
  12. Make appropriate referral of cases to other team members e.g. Health-Advisers for contact tracing.
  13. Participate in checking and signing of laboratory results
  14. Liaise with other health professionals and agencies (e.g. Gynaecology, General Practice, Breast clinic, Child Protection Unit) as appropriate and refer clients if indicated.
  15. Provide clinical support for nursing staff who are working to Patient Group Directives (PGDs) or who are nurse prescribers.
  16. Contribute to appropriate health promotion and education within the clinic setting.

Governance and General Administration:

The post holder will:

  1. Be involved in clinical governance and contribute to audit work across the service.
  2. Participate in meetings to discuss team changes, clinical practice and service development.
  3. Be involved in regular appraisal and demonstrate satisfactory performance and progress.
  4. Maintain effective working relationships within the multi-disciplinary team and associated agencies.
  5. Be punctual and not leave before the end of the clinic.
  6. Work in close liaison with other members of the multidisciplinary team.
  7. So far as is consistent with the proper discharge of duties, the post holder will be flexible and undertake to deputise from time to time for absent colleagues. This may include covering a clinic at a different site to that stated initially.
  8. Provide sufficient notice (approximately 6 weeks) for annual and study leave requests, and as far as possible co-ordinate arrangements with other medical staff to ensure an acceptable level of cover is maintained.
  9. Be expected to carry out all routine duties, such as letter writing and email etc within the job plan.

Subject to the provisions of The Terms and Conditions of Service you are expected to observe the HSCP’s agreed policies and procedures drawn up in consultation with the profession on clinical matters and follow the standing orders and financial institutions of the HSCP. In particular, where you formally manage employees of the HSCPyou will be expected to follow the local and national employment and personnel policies and procedures. You will be expected to make sure that there are adequate arrangements for hospital staff involved in the care of your patients to be able to contact you when necessary.

3.Review of Job Plan

Job plans are electronic and will be subject to review once a year by you together with the Clinical Lead for Sexual Health Fife. If it is not possible to agree a Job Plan with the Clinical Lead, either initially or at annual review, the matter will be subject to further review by the Fife Wide Division Clinical Director. If it still cannot be resolved, it will be reviewed by the Health and Social Care Partnership Associate Medical Director and subsequently the Medical Director NHS Fife, whose decision will be final.

4.PROFESSIONAL ACCOUNTABILITY

The professional accountability for the post rests with the Clinical Director Fife Wide Division and then to the Associate Medial Director Health and Social Care Partnership. The Medical Director for NHS Fife holds overall professional responsibility .

5. MANAGERIAL ACCOUNTABILITY

Dr Piegsa is the Clinical Lead for Sexual Health Fife and line manager for this post .

The Clinical Services Manager provides support to the department and represents the service at the Fife wide Division management team.

The Fifewide Division General Manager holds overall management responsibility .

6.MEDICAL EDUCATION

The post holder will be offered an opportunity to:-

  1. Participate in continuing medical education for professional development.
  2. Take part in local teaching and education events organised on by the department at an agreed and appropriate level e.g. DFSRH course.
  3. Contribute towards the education of medical students, junior doctors on placement within the service and other clinical attachments by mentorship, training and coaching at an agreed and appropriate level.
  4. If interested support will be provided to work towards obtaining the ‘Postgraduate Award in Medical Education (Sexual Reproductive Healthcare)’ [previouslyFSRH Letter of Competence in Medical Education (LoC MEd)].
  5. Provide training and, where needed assessment, for nursing staff within the department to expand their role within all aspects of sexual health care.

7.AUDIT

In collaboration with your Consultant colleagues you will be expected to participate in the audit of the work of the department. You will be provided with appropriate computer hardware and software along with maintenance of equipment and secretarial support for such an audit.

Experience in working with sexually active underage young people and knowledge of child protection issues and procedures.

9.TERMS AND CONDITIONS OF SERVICE

The post is covered by the Hospital Medical and Dental Staff and Doctors in Public Health and The Community Health Service (Scotland), Consultant Grade, Terms and Conditions of Service.

TYPE OF CONTRACT / Substantive
GRADE AND SALARY / Specialty Doctor: £38,685-£72,140per annum (pro rata for part time staff).
New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.
HOURS OF DUTY / Full Time 40 hours per week /pro rata to 40 hours for part time staff
SUPERANNUATION / New entrants to NHS Fife who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme. Should you choose to "opt out" arrangements can be made to do this via:
REMOVAL EXPENSES / Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.
EXPENSES OF CANDIDATES FOR APPOINTMENT / Candidates who are requested to attend an interview will be given assistance with appropriate travelling expenses. Re-imbursement shall not normally be made to employees who withdraw their application or refuse an offer of appointment.

FIFE REGION

The Region of Fife is bounded in the north by the Firth of Tay, in the east by the North Sea and in the south by the Firth of Forth. The Region spans an area of 130,700 hectares and has a population of 360,000. The population served by NHS Fife is currently around 280,000. There is a highly developed agricultural area in east and north-east Fife, and in the west there is an extensive cross-section of highly skilled and scientifically orientated industry. The largest towns are Dunfermline, Kirkcaldy and Glenrothes. The cathedral city of St Andrews is the seat of Scotland’s oldest and the UK’s second oldest university.
