Position Description

Position Title:Behavioral Health Nurse – RN

Reports To:Center Director

Classification:Hourly

Attendance at HIPAA and compliance educational programs is a requirement of employment.

SUMMARY:

Works with a multidisciplinary team of health care providers and provides comprehensive psychiatric registered nursing assessments, interventions, education, referrals and consultations for patients in the primary care setting.

DUTIES AND RESPONSIBILITIES:

  1. Performs psychiatric screening and triage with patients face-to-face or over the phone.
  2. Identifies individuals with risk factors and investigates possible associated disease (mental health) conditions.
  3. Formulates plan of care for clients.
  4. Refers to or consults with other health providers or resources based on assessment in order to prevent or resolve problems or concerns.
  5. Documents patient assessment/intervention data in medical records using established medical record forms and documentation practices.
  6. Provides patient education to individuals and groups about health maintenance and management of chronic disease process and mental health conditions. Communicates facts, ideas, and skill that change knowledge, attitudes, values, beliefs, behaviors, and practices if individuals and families.
  7. Follows universal precautions.
  8. Establishes an interpersonal relationship with individuals or families intended to increase or enhance their capacity for self-care and coping.
  9. Seeks information and generates optional solutions to perceived problems or issues through interactive problem solving with individuals.
  10. Participates as a member of a multidisciplinary team to facilitate teamwork, improve patient care processes and outcomes, and maintain an orderly work environment with efficient clinic flow.
  11. Responds to crisis situations according to policy.
  12. Demonstrates communication skills that are clear, effective, facilitate patient care and team functioning, and is respectful of differences.
  13. Utilizes supplies in a cost-effective manner.
  14. Performs clinic and outreach related tasks.
  15. Utilizes on-going assessment skills.
  16. Participates in Performance Improvement Program and Health Disparities Collaboratives.
  1. Establishes and maintains professional working relationship with other members of the health care team.
  2. Maintains professional competence by pursuing opportunities for continuous learning, attending continuing education offerings, reading professional literature, and engaging in other activities, which develop new knowledge and skills.
  3. Attends staff meetings.
  4. Maintains knowledge of, and complies with, personnel, nursing, medical, and clerical policies and procedures.
  5. Assists with seasonal programs as needed.
  6. Assists other team members.
  7. Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices.
  8. Performs other duties as assigned.

WORKING CONDITIONS AND PHYSICAL EFFORT:

Prolonged sitting or standing may be required. Those physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with receptionist duties in a clinic office setting will be performed on a repetitive basis. This also includes bending, twisting, reaching, lifting, pulling, pushing, and walking. The ability to distinguish letters and symbols as well as the ability to utilize telephones, computer terminals, fax machines, and copiers is required. Working under stressful conditions as well as irregular hours may be required.

QUALIFICATIONS:

  1. Graduate of a school of nursing accredited by National League of Nursing.
  2. Current license to practice as a Registered Nurse in the State of Ohio.
  3. One to three years full-time experience in registered nursing practice.
  4. Ability to work as a team member.
  5. Computer experience desirable.
  6. Ability to work with a culturally diverse group of people.
  7. Must be willing to work flexible hours as required by position.
