Main purpose of the role

As directed by the Lead Nurse, the post-holder will be an integral member of a multi disciplinary and multi-agency team offering:
  • Support to specialist prescribing and alcohol services;
  • Health and Wellbeing assessment;
  • Vaccination programme;
  • Naloxone programme as part of harm reduction;
  • Responsive titration as part of initiation to a prescription;
  • A range of clinical interventions to service users entering into the substance misuse treatment system, either through Open Access or the criminal justice system.

Role-specific responsibilities

Clinical Practice
3.1 / To ensure service users are titrated at initiation of prescribing on to a therapeutic dose using CGL titration process. The process will be incorporated into the health and wellbeing assessment and can be supported by other team members, including community Pharmacists, to ensure continuity of care.
3.2 / Work within and consolidate integrated care and treatment pathways for individuals who are infected with Hepatitis B and Hepatitis C and will provide testing and onward referrals for HIV.
3.3 / Provide screening and vaccinations for blood borne viruses, alongside other clinical and psychosocial interventions.
3.4 / Undertake health and wellbeing assessments for service users and ensure appropriate sign posting to primary care for identified health issues.
3.5 / Undertake phlebotomy, as required, ensuring adherence to identified pathways for blood sample labelling, collection, obtaining results and signposting. Use blood results as motivational tools and to assess current health status, the need for specialist medical referral and the suitability for community detoxification.
3.6 / Conduct assessments of physical dependency using a symptomatic withdrawal scale, using results to determine safe detoxification as required. Where indicated, to refer for specialist inpatient detoxification and to liaise with hospital and inpatient unit staff.
3.7 / To contribute to CGL Naloxone Strategy by training staff and service users in the use of naloxone . Ensuring naloxone is distributed to service users at every opportunity.
3.8 / Offer wound care assessment and basic wound dressing to clients. Organise signposting to other services as required, and ensure clear pathways into other treatment arenas as appropriate.
3.9 / Utilise psychosocial intervention, including ITEP, in both one-to-one and group settings to promote engagement in treatment services and ownership of Recovery Planning.
Communication
3.10 / Develop therapeutic alliances with people who have substance misuse problems and develop effective partnerships with service users to support their individual recovery journeys, engaging in key work sessions, offering health promotion and harm reduction advice.
3.11 / Adhere to Patient Group Directions and ensure requirements on updating are maintained.
3.12 / Engage proactively with both line management and clinical supervision and maintain a current knowledge of appropriate interventions. Ensure that all revalidation requirements are being met and communicated to line manager.
3.13 / As required develop and provide health education, especially in regard to problematic alcohol use and alcohol detoxification.
3.14 / Attend meetings appropriate to the development and maintenance of the role in agreement with and under the direction of the lead nurse.
Delivering a quality service
3.17 / The post-holder will contribute to the service to ensure that substance users consistently receive high quality services that adhere to the best practice guidelines and achieve high professional standards.
3.18 / Work flexibly across a number of operational sites as required and work flexibly within an agreed number of hours to maintain the most appropriate level of service provision. This will include evening and weekend working.
3.19 / Ensure that procedures are constantly reviewed to promote best practice and evidenced based practice in accordance with UK Guidelines on Clinical Management, NICE guidance, and CGL clinical protocols and procedures.
Leadership
3.20 / Provide support, coaching and teaching to staff working in the needle exchange around harm reduction and BBV.
Team Working
3.22 / To take part in assessing the suitability for transfer of clients to Shared Care services within the community.
3.23 / To work with others to ensure service users have timely access to safe and effective prescribing for community detoxification from substances, across operational sites.
3.24 / Contribute to the overall performance of the service to ensure that contractual output targets are achieved.
3.25 / Working with drug and alcohol workers to design and deliver a community detoxification plan utilising a care co-ordinated approach to clients care.
3.26 / Work alongside the designated Lead Clinician and Nurse Prescriber to formulate, plan and implement individual community detoxification programmes to ensure safety and optimise outcomes. This will include supporting service users’ detoxification from substances across operational sites.
Risk Management
3.27 / Provide harm reduction advice, information and equipment and ensure all equipment is disposed of safely.
3.28 / Utilise appropriate prevention techniques in order to reduce substance misuse related harm to aid in the prevention of multiple physical harms caused by drugs and alcohol.
Managing information
3.29 / Record and input client data and information in order that the service operates within contractual, administrative and financial requirements.
3.30 / Ensure that all data requirements in relation to your clients are met within defined deadlines.
