Congratulations! You have just completed phase I of your new employee onboarding: general orientation to the organization. You are now ready to go to your new workplace and team for phase II, your service- and job-specific orientation.
We want to be sure you have all the information you need to start your career here with Any Hospital USA. Please take a moment to reflect on the information that was reviewed during your initial orientation. We know you may want additional information or need to ask specific questions related to your own circumstances (e.g., benefits). We are here to provide that for you. Please complete this form and give it to your manager or supervisor and preceptor at your first meeting. They will either be able to provide you with the information you need or arrange for you to meet with someone who can.
