HAWAII Trip Permission Form

Dear Parents;

This page is written as a permission slip that you and your child must sign before he or she is allowed to participate in the Hawaii trip.

Included in the $2750.00 approximate fee that each student earned or paid is air and ground transportation, all but two meals, announced tours, and motel accommodations. The students are responsible for meals not indicated on the

schedule, and any extras they might wish to buy. They are responsible for about two meals.

The following are some of the rules for the trip:

1. Everyone follows the itinerary. No one will be given permission to travel with anyone else. We will travel as a group. NO EXCEPTIONS.

2. We will follow these hotel rules. As of midnight, or times designated, everyone will be in his or her assigned room and there will be no one in the halls.

After 12 a.m. the halls will be quiet. There will be room inspections at night. When there are

members of both sexes in a room, the door will

remain fully open. Sponsors have the right to

inspect rooms, suitcases, purses, coolers, etc., at any time. Room checks will be held twice nightly.Sponsors and hotel will enforce curfews

security staff. No one will be out of their rooms for any reason except emergencies.

3. No alcohol, drugs, or tobacco products of

any kind. Violation of this rule will result in the student's parent/Guardian’s providing and paying for immediate alternative transportation home.

4. Improper sexual behavior will not be tolerated. Student(s) violating this rule will be isolated and or sent home at their parent/guardian's expense.

5. Rooms should be left clean. Ashtrays, towels, etc. are not souvenirs. Damages to any individual room will be charged to those persons sharing the room. Any charges: phone, movies, etc. will be paid by the persons sharing the rooms. Long distance phone calls from the rooms may bepermitted with viable phone cards to bedetermined

6. All applicable rules from the Southwest High

School Student Handbook and Lincoln Public

Schools are in effect, as this is a school-sponsored trip and we are under that jurisdiction. We are representatives of Southwest High School and Lincoln Public Schools, and the United States and will act in an appropriate manner.

I agree to abide by all rules and guidelines set for the Lincoln Southwest trip December 28, 2017–January 3, 2018. I understand that this excursion is a school-sponsored activity and that all rules, regulations, and policies of Lincoln Public Schools, Lincoln, the State of Nebraska and the United States are in effect.

I understand that any infraction of these rules which may include disciplinary action not excluding exclusion from the group, notification of parents, and arrangements for immediate transportation home. I will conduct a representative of Lincoln Southwest High School.

It is understood that I give Mrs. Wilhelm, Mr. Lenz and/or LSW chaperones my permission to take necessary action in cases of misconduct or illness and will not hold them, Lincoln Southwest or Lincoln Public Schools responsible for accidents. I agree to provide andimmediately pay for transportation home for theabove student if their actions warrant suchdiscipline.
