GulfstreamContractor Pre-Qualification Requirements

Purpose / To hire Contractor firms for The Dow Chemical Company Sites that have proactive EH&S programs and processes in place and to drive worksite incident rates to their lowest possible point.
Scope / This applies to all Contractor companies seeking work on Dow Chemical Sites.
Policy / Contractor companies seeking work on Dow Chemical Sites must meet specific Safety, Health, and Environmental standard requirements.
Pre-Bid Process / Prior to sending out bid packages to potential contractors, a pre-screening process will be followed in order to determine if a company can be considered for bids.
The pre-screening process will require companies to submit the following documentation to the appropriate EH&S contact:
  • Previous 3 year OSHA 300 and 300A logs
  • Current year OSHA 300 and 300A log
  • Previous 3 years EMR data
  • Current Health and Safety Manual
  • Current Substance Abuse Policy with test panel
A review of the above documentation will occur with the Gulfstream EH&S Leader who will give final approval of acceptable companies based on their safety performance. These companies will then be allowed to bid on work for Gulfstream.
Technical considerations may at times override safety performance and must be agreed on ahead of the bid process.

Gulfstream Contractor Pre-Qualification Requirements

Pre-Work / Once a company has been selected to conduct work for Gulfstream,
The contract firms must complete a standardized form that indicates an overview of their Safety, Health, and Environmental programs and policies.
This is a standard form established by the Houston Business Roundtable (HBR) and is used by many businesses in the area. It is accessed from the Houston Area Safety Council (HASC) and is called “XNET”. All pre-qualification documentation including the documents listed below must be posted in XNET.
Contractors must post the following documents in XNET:
  • Completed Pre-Qualification form
  • Current Substance Abuse Policy with test panel. Posted annually.
  • Current Health and Safety Manual.
  • OSHA 300 and 300A logs for the previous 3 years.
  • OSHA 300 and 300A log after 1st quarter of current year.
  • EMR data for the previous 3 years
Contractors must then grant access permission to The Dow Chemical Company, Freeport and companies acting as an agent of The Dow Chemical Company.
XNET, the web based Contractor Pre-Qualification database is available through the Houston Area Safety Council.
Upon review of the contractor Pre-Qualification information, acceptable companies will be granted access to the Dow site for work.

Site Access Approval/Denial / The types and duration of site access approvals are listed below
Type / Definition
First Pass Approval / For the last three calendar years the firm must have had:
  • 1 or less OSHA recordable injuries / illness per year or an average OSHA rate of less than or equal to 1.0.
  • A downward trend in individual OSHA rate per year for the last three calendar years with the most recent year being less than or equal to 1.0.
  • an average EMR of 1 or less.
Meet minimum expectations as outlined on the HBR Pre-Qualification form including:
  • a written Safety and Health Program
  • a written Substance Abuse Policy
After meeting these criteria a contractor firm is authorized, from a Safety perspective, to work on site.
Requires review of all pre-qualification information on a 3 year basis.

Probationary / Companies that cannot meet the criteria above may be accepted for work on the site on a probationary basis if the following conditions are met:
  • for the last three calendar years the firm has had 2 or less OSHA recordable injuries / illness per year or an average OSHA rate between 1.0 and 2.0.
  • A downward trend in individual OSHA rate per year for the last three calendar years with the most recent year being the best performance.
  • an average EMR of 1.0 or less over the three year average.
  • meet or exceed the expectations outlined on the HBR Pre-Qualification form including:
a written Safety and Health Program
a written Substance Abuse Policy
  • or have an action plan in place to meet minimum expectations. (The company must submit an Action Plan outlining specific measurable goals and plans for continuous safety improvement.)
Requires review of all pre-qualification information on an annual basis until such time that the contractor company reduces their OSHA Rate and/or EMR rate to the site acceptable limits.
Denied / Companies that cannot meet the criteria above will not be accepted for work on the site.
Exceptions / Potential exceptions will be reviewed with the GulfstreamEH&S Leader and the appropriate 3rd party HSE Manager (if applicable) for approval.

Site Access Restrictions / Access to the construction sites may be restricted based on a company’s EH&S performance and is subject to Owner approval.
