GUIDELINES for INSTRUCTOR MAKE-UP TESTS
For your convenience, Assessment Services offers make-up exams to students who miss classroom tests. This offers greater flexibility for DSC students when a make-up exam becomes necessary. Please be aware, seating is on a space available basis and staffing is limited; therefore Assessment Services cannot accommodate entire class testing or administer Online course exams.
Instructor procedures:
· Access Instructor Make-up Test Administration Form via Assessment Services webpage or in the Assessment Services Center
o Complete the above form providing all requested data and attach to tests prior to submission
· Deliver exams in person to AS office or use Instructor Make-up Testing Proxy Form **
· Instructors will provide photo ID upon request (staff are not always familiar with faculty)
· Use electronic sign-in station to enter required data for Each Course/Test # being dropped off. ALL test copies MUST include the following information:
1. Instructor’s Name
2. Course Name and Number
3. Test Number
4. Student’s Name on each exam (or student must have an instructor signed test pass).
Instructors can pick up test passes at Assessment Services
· Reusable exams will be limited to three (3) copies per course test.
· Test copies must be clean, undamaged and updated.
· Tests will be picked up and signed for on the Completed Instructor Tests Sign-Out Log by instructor (or authorized proxy)
· To remove untaken tests, instructors complete the Untaken Instructor Tests Sign-Out Log
· Instructors will pick up all of their make-up exams during finals week of each term
***Beginning in the 2014 Academic year, mechanical pencils are no longer permitted.***
Pencils will be provided for testing convenience. Please inform your students of the new guidelines.
Student procedures:
· Students must have a photo ID to test and will be required to sign-in
· Student must know their instructor’s name when testing
· Test passes are required for make-up exams without student names on them
· Students will follow AS center rules for testing times (please check at each campus)
Assessment procedures:
· Post forms relevant to Instructor Make-up Testing on Assessment Services webpage
· Provide security for instructor exams using sign-in and sign-out processes
· Administer tests per instructions
· Send exams to other DSC sites: AS staff will log in exams at one campus, package securely, and send to AS Centers at other campuses via campus mail. Receiving campus will log-in and secure the tests
· Coursework will not be collected or accepted
· Assessment Services does not score or distribute scores for instructor tests

**Faculty may authorize a proxy from their department to deliver and/or pick up exams by submitting the Instructor Make-up Testing Proxy Form. ***Daytona State College identification must be presented.
