Grant Guidelines

Purpose

The purpose of the Give ‘Em Five Fund is to provide a source of additional funding to assist schools

in planning, developing, and implementing innovative programs and creative opportunities which will result in increased student achievement. Hopefully, this will allow educators additional opportunities to help students grow and reach their potential.

Selection of Criteria

A team composed of community members will review all applications and use the following point system to evaluate each application. The criteria are as follows:

Needs Statement 20 points

Purpose & Objectives 15 points

Program Description 30 points

Student Involvement 10 points

Creativity 15 points

Evaluation 10 points

Funds may be used for:

Equipment (playground, sports, band, etc.)

Books (other than textbooks)

Computer Software (no hardware)

Student supplies

Local field trips

Educational field trips

Other needs will be considered as well

Timeline

Applications can be sent at any time. Grants will be awarded annually. Deadline for accepting grants is October 15 of each year.

Applications

Applications should be typed using the actual grant application format. If additional space is needed other than what is provided, applications may be typed out on a separate paper as long as the headings are listed. Be specific as possible in providing an itemized budget request. List the items, the company or companies that supply the items, and the cost. Be sure to include all possible charges, shipping and handling, installation fees, logo prints, etc.

The grants that are awarded will be notified by phone/ mail and all purchases approved in the grant request will be purchased and delivered to the school.

Grant recipients are expected to complete a simple report at the completion of their project to evaluate the effectiveness of their grant.

Discretionary Funds Grant Application

NEEDS STATEMENT (20 points)

Establish the need for the project by stating statistical evidence, statements from authorities or other supported data.

PURPOSE AND OBJECTIVES (15 POINTS)

Define the objective(s), eventual outcome, and overall benefit of your proposal.

PROGRAM DESCRIPTION (30 POINTS)

Give a clear and detailed description of your proposal. Include details of activities which support the overall outcome of the project.

Student Involvement (10 points)

Describe the level of student involvement and participation in the project. Describe how students will ultimately benefit by the project.

Creativity (15 points)

Describe how this project challenges or stimulates the students.

Evaluation (10 points)

Describe how you will determine if your objectives have been achieved, and whether or not the project is successful.

Budget

Detail your budget request. Itemize the kinds of materials and equipment needed. List the company supplying the items and please include shipping costs.
