AQUATICAPERMISSION FORM

permission to attend Aquatica for this year’s senior trip. Students must be at the school no later than 9:30 am. They will be searched according to Aquatica’s policies at the school. The event at Aquatica ends at 2:00 pm, at which time the students will be bussed back to the MDHS. All students are required to leave campus by their own vehicle or be picked up by a parent/guardian within ½ hour of return.

Students are required to follow the MDHS student code of conduct and abide by Aquatica rules and procedures. Any student caught at Aquatica doing something inappropriate may be detained in a holding station by Aquatica’s personnel for the remainder of the day. At the end of the evening they will be picked up by a MDH chaperone and escorted to the bus. No refunds will be given to these students and they may also incur disciple action at MDH depending on the severity of the infraction. Please be aware that if the matter is severe enough the Orange County Sheriff’s Office will handle the matter and you will be notified promptly.

AQUATICA GUIDELINES

Meet in the SENIOR LOT at no later than 8:45 am on Tuesday, May 16th. You may park in the senior lot.

You must have your permission slips in on time!

Tickets are non-transferable. This means it is only for you and you may not have someone else go in your place because you changed your mind. If you end up not being able to go for a valid reason, see Ms. Goldsteinprior to the event.

No student may drive to Aquatica. You must ride down and return on the bus. NO FOOD or DRINK on the bus!

Be prepared to be “lightly” searched here at school. (Girls – you must open your purses and show us the contents - a female chaperone will do this.)

You will not be admitted on the bus if: (NO REFUNDS)

  • You are not properly dressed – Be a lady and a gentleman
  • There is any indication that you are in an altered state.
  • You have alcohol or tobacco products with you.

Remember you may not bring the following – they will be confiscated before you get on the bus:

  • Hats, Bandannas, Do-rags, etc (you may not have them period!)
  • Laser pointers, knives, cigarette lighters, etc
  • Aerosol cans
  • Medications of any kind. If it is something that you have to take with you, you may have a chaperone carry it for you.

Bring:

  • Towel
  • Sunscreen
  • Wear clothing over bathing suit during travel
  • Money (you get one meal ticket)
  • Change of dry clothing
