I do hereby permit my child to participate in the Good Shepherd Swimming Program. I understand the purpose of this program is to teach sportsmanship and physical development through organized sports activities. I further understand and agree with the right of the Good Shepherd Sports Committee to suspend (without refund) my child for unsportsmanlike conduct. TO THE BEST OF MY KNOWLEDGE MY CHILD IS IN GOOD HEALTH AND ABLE TO PARTICIPATE IN THIS SPORT. IF I CANNOT BE REACHED IN AN EMERGENCY, THIS IS WHAT I WISH YOU TO DO:
ADDENDUM TO GSSP BYLAWS:
GSSP GENERAL MEMBERSHIP (includes G.S.S.C. members, Coaches, Volunteers, Parents, Players- members and fans) are representatives of Good Shepherd Sports. As representatives, they are responsible for adhering to the Mission and Objectives of the Good Shepherd Sports Program. Any member of the general membership who has been deemed to have acted contrary to the goal and objectives of the program shall be notified in writing. If these actions which are contrary to the goals and objectives continue, any or all of the following actions may be taken
(1) removal from GSSP General Membership; (2) A permanent ban on their attendance from all Good Shepherd home and away athletic contests; (3)Expulsion from the sports program of the child for a specific period of time (to be determined by the GGSC, but not to exceed the balance of the season he/she is currently participating in): and (4) Suspension from the sports program for (1) one year from the date of the incident.
