PERFORMANCE MEASURES - Supervised Visitation Services

Pursuant to Texas Human Resources Code §40.058 all contracts for client services must include clearly defined goals and outcomes that can be measured to determine whether the objectives of the program are being achieved.

Outcome #1: Authorized services provided by the Contractor meet Caseworker expectations.
Outcome Performance Period: Contractor performance for this outcome is determined for one or more of the following performance periods, wholly or partially, depending on the contract start and end dates: September 1 through November 30; December 1 through February 28/29; March 1 through May 31; and June 1 through August 31.
Outcome Indicator: Percent of aggregated favorable responses made to items in the Caseworker Satisfaction Survey Questionnaire
Outcome Target: At least 90%
Purpose: The purpose of this measure is to evaluate the Contractor’s overall quality of service, as perceived by CPS Caseworkers.
Data Source: Caseworker Satisfaction Survey Questionnaires obtained by DFPS from a sample of CPS Caseworkers
Methodology: The numerator is the aggregated number of favorable responses to the Caseworker Satisfaction Survey Questionnaires obtained from Caseworkers for the outcome performance period. The denominator is the aggregated number of all responses, excluding N/A responses, to the Caseworker Satisfaction Survey Questionnaires obtained from Caseworkers for the outcome performance period. The numerator is divided by the denominator, multiplied by 100, and stated as a percentage.
Outcome #2: Authorized services provided by the Contractor meet Participant expectations.
Outcome Performance Period: Contractor performance for this outcome is determined for one or more of the following performance periods, wholly or partially depending on the contract start and end dates: September 1 through November 30; December 1 through February 28/29; March 1 through May 31; June 1 through August 31.
Outcome Indicator: Percent of aggregated favorable responses made to items on the Supervised Visitation Satisfaction Survey Questionnaires
Outcome Target: 50%
Purpose: The purpose of this measure is to evaluate the Contractor’s overall quality of service as perceived by adult Participants.
Data Source: Contractor self-reported data
Methodology: The numerator is the aggregate number of favorable responses from Supervised Visitation Satisfaction Survey Questionnaires collected during the Performance Period. The denominator is the aggregate number of all responses from Supervised Visitation Satisfaction Survey Questionnaires collected during the Performance Period. A response of N/A or an item with no response is excluded from the denominator. Divide the numerator by the denominator, multiply by 100, and state as a percentage.

Performance Measure Requirements

The Contractor will be responsible for supporting the collection of performance measure data. The Contractor shall:

1. Distribute a Supervised Visitation Satisfaction Survey Questionnaire to evaluate the level of satisfaction Adult Participants have with the visitation arrangements and the Contractor employees they interacted with. The Supervised Visitation Satisfaction Survey Questionnaire the contractor is to distribute for purposes of the performance measure is identified as Form 3114 and is available for reproducing on the DFPS public website. The contractor employee who is monitoring the last of the visits authorized by an individual 2054 will distribute the Supervised Visitation Satisfaction Questionnaire to the Adult Participants at the end of the last scheduled visit. (Note: Additional visits may be necessary; if so, they will have a new 2054 authorization, and the Supervised Visitation Satisfaction Survey Questionnaire will be distributed again at the last visit scheduled from the new authorization.) The Supervised Visitation Satisfaction Survey Questionnaire should be filled out by the Contractor before distribution with the name of the Contractor employee monitoring the visit, the name of the Contractor, and the date of distribution. Completion of the form is voluntary, and Adult Participants should not be required to give their names.

2. Keep all Supervised Visitation Satisfaction Survey Questionnaires received on file and available to DFPS upon request for the time period specified by DFPS for records maintenance. The Supervised Visitation Satisfaction Survey Questionnaires must be maintained in a manner to allow for the testing of the validity of the results being reported. This means that documentation must be maintained for each Performance Period consisting of copies of the dated Supervised Visitation Satisfaction Survey Questionnaires, and a copy of the performance results which were reported to DFPS Contract Performance. (Neither the original nor copies of the Questionnaires should be sent to DFPS; only the Aggregate results.)

3. Tally the Supervised Visitation Satisfaction Survey Questionnaire results. Results to be reported to DFPS Contract Performance will be the Aggregate number of responses to items 1 - 6 from all the Supervised Visitation Satisfaction Survey Questionnaires collected during the Performance Period being reported, excluding blank and NA responses; and the aggregate number of favorable responses to items 1- 6 from all Supervised Visitation Satisfaction survey Questionnaires collected during the Performance Period being reported.

4. Report the results for each Performance Period in the format specified by DFPS. DFPS has developed PMET (Performance Management Evaluation Tool), an Internet-based data collection and reporting system for Contractors to self-report performance measure data.

5. Report the results for each Performance to DFPS Contract Performance using PMET. If the Contractor has never reported performance data for this contract using PMET, an account must be registered prior to reporting the data for the first Performance Period. The Contractor TIN (Texas Identification Number) and the Contract Number are needed to register. Instructions can be found at https://www.dfps.state.tx.us/application/PCSPMET. 6. Comply with report date time frames. Performance Measure reporting is to be entered into the PMET system within 30 days of the end of the Performance Period in accordance with the table below:
