Oklahoma Transition InstituteTeam Goals Tool

Use this tool to document your team’s transition goals for the coming year. Some goals have a more rapid implementation than others, so this may be a short term, or a long term plan of action. The action steps are the primary steps that need to be taken in order to achieve the desired goal. It is desired that a Transition Team have two to three goals for a program year, however in some cases it is a big task to get even one new program, or change in place for a school site. Goal 1: Getting buy in from Superintendent to sign contracts with OTI

To be implemented by date:

Action Steps:

  • Actively seek out stakeholder (parent) buy in to making OTI services available in local schools.
  • Reach out to parents – PT meeting – Booth
  • Going to the schools and getting more involved in getting specific details on how quickly the school would be reimbursed $$.
  • Contact the board to identify how quickly payment is being made – show proof to superintendent of how quickly they will get reimbursed – provide examples.
  • Showing the superintendent visual proof of how quickly the payment can occur.
  • Helping with giving parents an outlet to advocate for the rights of their children. It only takes one parent to identity a need that isn’t being met by the school that should be.

Goal 2: Develop a Transition and skill development Camp

  • Develop a name 
  • Gain access to the WOW google doc examples – (Working on the Wichita) don’t reinvent the wheel
  • Reach out to local Donors/resources
  • Reach out to local businesses
  • Restaurants
  • Reach out to local celebrities
  • Reach out to local Churches/volunteers
  • Reach out to local boy scouts and girl scouts – integration
  • Reach out to local tribal DRS
  • Fire fighters
  • CPR/EMT – life guard
  • Reach out to Courtz Mountain Lodge – job opportunities @ the Lodge?
  • Rotate with each job to help identify what jobs are available at the Lodge
  • 8-10 kids – application process/screening – criteria, expectations etc.
  • Identify list of activities – connect them to job resources and living skills
  • Look at other camps that have been hosted at the same location – what worked? What didn’t work?
  • End of the week – all the donors would be invited to the event – kids would plan the event, work the event “Thank you Dinner” – invite the parents (bring together, parents, children and community/donors)
  • Thank you letters to all donors after the event – maybe send a group picture in the cards
  • Poster making – activity associated
