ASPIRE – Check in

Getting ready for the final 20 months

To reenergize ASPIRE youth, families and staff, targeted activities will be implemented to prepare for the final 20 months of ASPIRE. These activities begin on October 6, 2017 with this ASPIRE Check and continue through May 31, 2019 when Case Management ends. This ASPIRE Check In will occur between October 6, 2017 and January 31, 2018 for ASPIRE youth/families.

October 6, 2017 through January 31, 2018

It is critical that youth and families who enrolled in ASPIRE are reminded of the important role they play in the PROMISE research study, and are inspired to continue or increase their participation. It also includes a renewed effort by Case Managers to connect with ASPIRE participants in meaningful ways.

Check-In with ASPIRE Youth and Family

During at least one monthly case management meeting between October 6, 2017 andJanuary 31, 2018, engage in a refreshed, reenergizing dialogue with families. This intentional conversation is not a substitute for a monthly case management meeting. Rather it is expected that this ASPIRE Check-In will be included in the typical case management meeting. The ASPIRE Check-In is a formal, intentional conversation with the goal to improve full participation in ASPIRE and the development of ASPIRE SMART goals. All monthly case management meetings during this time period will still be held.

Case Managers will prepare for the ASPIRE Check-In and then complete an “ASPIRE Check-In Form” during the meeting, recording the details of the conversation in the General Case Note afterwards, updating the ASPIRE Services Plan and Assessment. The ASPIRE Check-In includes the following:

  • Reminder that ASPIRE is a research study. It includes
  • Two groups: ASPIRE Services and Usual Services
  • This youth/family are in the ASPIRE Services meaning they are provided more services than a typical youth or family
  • Purpose: to learn if these extra services are provided to enrollees, will result in improved education, becoming employed and becoming more independent in the future
  • Enrolled in ASPIRE on ____(date) and agreed to participate in what ASPIRE will offer.
  • ASPIRE was originally intended to end on September 30, 2018, but the Department of Education is allowing us to continue services until May 31, 2019.
  • ASPIRE wants to be sure they youth/family are connected to what they need both now and in the future, and have the knowledge and skills they need.
  • What has the youth/family participated in since enrollment?
  • Gather data from AIMS. Include both case management and provider interventions.
  • Review youth and family goals and tasks up to this time – achieved, active and discontinued.
  • Discuss each and the reason each continues or not.
  • Are they still important? Is it time for new goals/tasks?
  • Ask how their participation can increase. What can ASPIRE do to support increased participation?
  • It’s important to the future that to participate in case management services and attend interventions and workshops
  • Specifically plan your next meeting and what the Case Manager will do to follow up on this discussion. What will be new or different?

This intentional conversation will be documented in the General Case Note of the Case Management Contact instrument for each SID, as well as the ASPIRE Services Plan and Assessment Instruments. Additionally the “Check-In Form” will be completed and uploaded into the individual SID “Documents and Attachments” instrument.
