**** Get some adult help with all these forms!!!!!

Science Fair Applicants

  1. For all forms, print neatly!!!!!
  2. Fill out Form (1) with your name. (Your teacher will sign and date!)
  3. Fill out Research Plan (1A). Note that some of the dates have been filled in for you.
  4. Fill out Research Plan Attachment – please type A, B, C, and D– on sheet, you can say, “see attached” and then just print it off your computer and staple

P.S. When you type your C. Description, you can do bullets, but put all steps in

the future tense, as if you hadn’t done them yet! i.e. I will…….

  1. Please print a copy of your abstract and attach that to the Research Plan Attachment
  2. You sign and have a parent sign Approval Form (1B). Note that the dates have been filled in for you.
  3. Then fill out the yellow and blue forms, printing neatly please. You may attach a copy of your bibliography by stapling it to the blue form.
  4. Return these forms to your teacher by Monday, February 11.

VJAS Students

1. Fill out Forms 1, 2, and 3 (be sure to fill out Certification form on back of page one if you used either humans or vertebrate animals in your experiment.) Please type or print very neatly using black ink

2. Please write the Category letter(s) where it says “Code” at the top left of Forms 1, 2, and 3. If you are unsure as to which category it would be most advantageous for you to select, please see your teacher first! before xeroxing your copies.

3.. Put Forms 1, 2, and 3 on top of a copy of your paper minus your title page) then xerox 4 copies. (Don’t forget – your Form 1 has two sides. Both sides need to be xeroxed even though you didn’t have to fill the Certification form out!). Staple, using a large stapler, One Time!!!each of the five copies.

4. Write “MASTER” in red ink at the top of original copy

5.. Rubberband all 5 copies and bring them to your teacher along with a check for $6.00 (not cash) written to H-B Woodlawn

6. Absolute Drop DEAD due date to your own teachers – Thursday, February 21.

Teachers!

Here are the Forms for everything – Science Fair and VJAS. Please xerox copies as needed. The only colored ones are two of the Science Fair forms, but I hope I’ve given you all that you need of those.

Following are some important things to remember.

  1. I have predated the Science Fair forms – you all should know this routine by now – but, remember to sign your names in the appropriate spots for the adult sponsor before you xerox copies and hand them out (saves a lot of time correcting after the fact)
  1. Make sure the students know that when they are writing their procedures – that to somehow take them out of the past tense (sometimes I just get the kids to do bullets of the procedures for these official forms)
  1. Hopefully, you can explain if any parent questions the meaning of the dates already on the forms.
  1. With the VJAS forms, make sure when you xerox, AND WHEN THEY XEROX THEIR COPIES, that both sides of page 1, the Entry Form, even though they might not need to fill out any of the info on the Certification form
  1. Note: If someone is applying to the Mathematics and Statistics category for VJAS, I’ve included the “different” pages 2 & 3 of the VJAS forms, those especially are for that category.
  1. The VJAS papers and checks go to Dave, the Science Fair forms come to me.
