This year we are offering up to 6 scholarships for post-secondary students (from technical school to graduate school) currently enrolled a Health Information Technology program (or Health Informatics program or closely related program) in an accredited school in the state of Georgia. The amount of each scholarship will be between $2,000 and $3,000, depending on total funds raised and number of selected recipients. All recipients will receive equivalent amounts.
About Georgia HIMSS
The Georgia Chapter of the Healthcare Information Management and Systems Society (GAHIMSS) provides statewide leadership for the advancement and management of healthcare information and technology. Collectively, HIMSS represents more than 40,000 individual members across the country – with GAHIMSS encompassing over 1600 healthcare IT professionals – including many key CIOs from the state’s largest hospitals and healthcare facilities. GAHIMSS is dedicated to improving Healthcare IT locally through advocacy, collaboration and education, and to ensure quality patient care throughout the state of Georgia.

Scholarship Application Instructions
Please complete this application and submit along with two reference letters (all submitted in the same e-mail, please) to the e-mail address listed below.
One of the two reference letters MUST be from an instructor, professor, or program coordinator /director in the health information technology (or HIT or health informatics related) program that you are currently enrolled in.
We do not require applicants to send transcripts, proof of employment, or separately sent reference letters (i.e., reference letters do not need to be separately signed and sealed and can be copied directly into this application or attached to the e-mail which also includes the completed application). However, Georgia HIMSS and the Scholarship Review Committee reserves the right to verify current academic enrollment, verify positions held or mentioned in the application, and/or to contact references directly to verify reference authenticity. Winners will be asked to provide official academic transcripts.

Requirements
- Scholarships are available to students at any level of post-secondary education (technical school to graduate school)
- Students must currently be enrolled at an accredited post-secondary school in the state of Georgia and the program currently enrolled in must be Health IT or Health Informatics related.
- Scholarship recipients will be asked to attend the Georgia HIMSS Annual Golf Tournament in May 2016 (for recognition and may be asked to volunteer at the event) and will also be asked to volunteer in at least two (2) Georgia HIMSS events in the following year (i.e. sometime between May 2016 and May 2017).
- Scholarship Applications will be evaluated according to the following rubric (minimum of a total of 80% required):

b. Scholarship – 30%
c. Vision – 20%
d. HIT Experience / Engagement – 10%
e. Georgia HIMSS Current or Future Commitment – 10%

Please include a copy of the reference letters, which should include contact information for each reference, either copied-and-pasted into this application or in PDF or Microsoft Word document format in separate documents that can be attached to the final e-mail submission along with the completed application to the e-mail address above.
