Functional Behavior Assessment Cheat Sheet
Medical Interventions / N/AEnvironmental Interventions / · J. Jones will check in with the school counselor daily in the morning when he arrives at school and in the afternoon. The school counselor will give J. Jones his daily sheet and discuss his goals for the day.
· J. Jones will work with the school counselor on social skills lessons. He may work individually with her in the office, and then practice these skills in the classroom with another peer model (designated by the teacher).
· J. Jones will carry his daily sheet around all day. After each activity, his teacher (including special area teachers) will rate his behavior using a smiley face scale (see attached).
· A Social Story will be created for J. Jones to use when he gets upset. He will need to be taught to get the book and how to use it to calm down. A “chill out” spot or safe place will be designated for him to take some time to de-escalate.
Curricular Interventions / · J. Jones’s teacher will use Behavioral Momentum (giving 2-3 simple directions and then giving the cue to begin work) to help J. Jones get started on assignments. For example, J. Jones’s teacher might say, (1)”Hold your pencil up in the air.” (2) “Put your name on your paper.” (3) “Begin problem number 1. Go!”
· Implement choices throughout J. Jones’s day. Provide him with a choice of what part of the assignment he wants to do first. Or, if there are 2 worksheets or 2 activities, provide him a choice of what to start on first. He still has to do both of them, but this provides him more ownership.

Replacement Behavior Interventions / Attention/Escape
· J. Jones’s teachers will praise him when he displays positive behaviors and self-control.
· J. Jones will carry his daily sheet with him throughout the day. After each activity, his teacher will rate him on his behavior of (1) I was safe with my hands, feet, and objects. (2) I used positive words with my teacher and peers. He will receive natural attention from his teacher after each activity when they discuss J. Jones’s behavior.
· J. Jones will earn smiley faces for good behavior after each activity. After earning a pre-determined number of smiley faces, he can choose a reinforcer (e.g., computer time, visit Mrs. Stewart, visit the office, help the teacher, etc.) from a reinforcer menu.
· J. Jones will also receive attention from the counselor at the end of the day when reviewing his daily sheet.
