FRESNO HCAP

Fresno Healthy Communities Access Partners (FHCAP) is a thirteen-year-old nonprofit organization of eleven health care and community organizations working together to improve access to health care for medically underserved communities in Fresno and the San Joaquin Valley.

HCAP is envisioned to serve as a foundation for greater community cooperation to meet health needs. Through coordination, collaboration, implementation and resource allocation, decisions can benefit the health care of all Fresno County Residents. Our board members represent the local safety-net health care delivery system.

Fresno HCAP provides leadership in the following collaborative projects:

  • Program Administrator for Children & Families Health Initiative for Fresno County (CHI-FC), a 17 member community and county coalition operating since 2005 to ensure that all children and their families living in Fresno County have access to a medical, dental and vision home. FHCAP is a member of California Coverage and Health Initiatives (CCHI) and Community Health Councils’ Covering Kids and Families.
  • Leader for Certified Application Assistor/Certified Enrolment Counselor(CAA/CEC) coalition. OERU (Outreach, Enrollment, Retention and Utilization) services are coordinated for low income Fresno populations working with 14 county and community-based partners including:

  • Advisor to Insure the Uninsured Project (ITUP): ITUP is an independent nonprofit organization and health policy institute that for more than two decades has offered expert analysis and facilitated convenings of California health leaders on emerging issues affecting health and health policy in the state.
  • Board member of California Coverage and Health Initiatives (CCHI):CCHI is a statewide association of outreach and enrollment organizations that focused on helping children and families navigate into affordable health coverage and have access to high quality health services.
  • Member of Covering Kids and Families (Community Health Councils), a statewide coalition of advocates for access to and utilization of health care services for low income Californians.
