FREQUENTLY ASKED CARF QUESTIONS

Below are examples of open‐ended questions surveyors may use to gather information on conformance to standards. Notice similar questions are asked in different ways in an effort to determine program consistency/continuity of care.

Surveyors may ask some of the following questions to the person served (clients):

Tell me about your experience (at/with) the (facility/center/program).

Describe for me your experience in the program.

Tell me about your care at (program name).

Tell me about your typical day here at (program name).

What is your usual day like during the week? On the weekend?

Preadmission/admission questions:

Tell me the story about how you came to this facility/program.

Tell me how you came to be here and how you are being helped.

How did you learn about this program?

How did you decide this program was right for you?

Why did you choose this facility for treatment?

Please describe to me the process when you first arrived to the program.

Admission Process:

What was your first day here like?

Tell me about your experience with the team, including involvement in the plan development.

Treatment plan/ inclusion of the Persons Served

Talk about your individual plan of care and goals.

Explain how the goals were determined.

Tell me how you provide input into your plan of care.

How are you included as a member of the team?

How have you been involved in setting your goals?
