Important Guidelines for Filing all Forms:
All forms have a caption located on the top of the first page.Fill out this section to include the county of filing,your names as they appear in the order,your case number and the division. Without this caption completed, the court is not responsible for accepting your filing.
All documents have a Certificate of Service section located at the end of the document. Complete this section when only one person signs (example: Sworn Financial Statement and Certificate of Compliance). Include the date you provided a copy; how (by hand delivery or US mail) you provided the copy; and the name and address of the person you provided the copy (the other individual in your case); your signature.
Notary: You may use any public notary service or the court will stamp your signature upon providing a valid photo ID to the court. If you use the court, there is a $.75 charge for copies.
Important:make two copies of all documents, one for yourself and one for the other party, before filing the original with the court.
If at any time parties cannot reach mutual agreements, mediation attendance is required. Pursuant to §13-22-311(1), C.R.S., the parties may select their mediator from private mediators, mediation organizations, or from the Colorado Judicial Branch Office of Dispute Resolution at

Sworn Financial Statement JDF 1111 and, if needed, Supporting Schedules JDF 1111 SS
Each person fills out their own form. Do not attach financial documents referenced in this form. Deadline for filing: 42 days from date Petition was filed or date papers were served.
Certificate of Compliance with Mandatory Financial Disclosures JDF 1104
Each person fills out this form. Any financial documents referenced in this form is exchanged with each other but not filed to the court. This document is filed at the same time as the Sworn Financial Statement. This form does not require a notarized signature.

Certificate of Completion of Parenting through Separation/Divorce Class
This certificate is given to you at the completion of the class. Upon receiving your certificate of attendance, write your case number on the certificate and file with the court. Deadline for filing is 49 days.
Parenting Plan JDF 1113
Both parents fill this form out together if possible. Be as specific as possible to provide clarity of your plan. Both parents provide notarized signatures on this document. If you are not in agreement and cannot sign one Plan, mediation will be required. Contact Laurie Mactavish, the Family Court Facilitator for further direction if this might be the situation in your case.
Child Support Worksheet JDF 1820 E
This worksheet is in an Excel format and, like the above forms, can be found on our website under “All Domestic Forms.” You will need the following information: gross monthly income of both parents as reflected in your Sworn Financial Statements, monthly cost of work related childcare (averaged over 12 months), monthly health care premium(s) for the child(ren) only, and the yearly estimated overnights with each parent based on your agreement in the Parenting Plan. After completing your names, child(ren)’s names, case number and correct court location, hit return and scroll down to bottom of the worksheet and print as directed. Sign and date completed worksheet, and file it with the completed Support Order as described below.
Support Order JDF 1117
This form is completed by filling out the caption and the information for Petitioner, Co-Petitioner/Respondent, and information for your child(ren) on the first page.Leave remaining questions blank as this if for the Judge to complete. One form only is needed.
Note: Every case is scheduled for an initial status conference with the Family Court Facilitator. Unless you have been scheduled close to the 42 day deadline, no forms are due before the conference. If you would like to complete all forms, you may use the conference for a document review before documents are filed. This last step saves you significant time.
It is advised to schedule a document review after your agreements have been reached (Separation Agreement and Parenting Plan) BEFOREfiling to prevent the need to amend documents if incomplete. Contact Laurie Mactavish, Family Court Facilitator, to schedule.
