This form is completed in conjunction with an Annual Performance Review and Planning Form for Academic Staff. It is to be used for Formal Probation Reviews as follows:

  • Mid-point Probation Reviews: completed at year two (2) of a five (5) year probation period (Note: Continuing staff whose probationary term is for three (3) years or less are not required to complete a Mid-point Probation Review).
  • Final Probation Reviews: normally occur in the final year of the probation period.
