Form TC-59a(hr)APPLICANT/RESIDENT INTERVIEW

This Interview form must be used for an applicant/resident household under the following circumstances:

  • When the original rental application has been completed and the household is being pre-screened for an apartment – this Interview is used to determine what verifications may be required to prove income, allowances and any other relevant eligibility information. Most of the form numbers have been listed.
  • When the household is applying for HUD Section 8 Subsidy or USDA-RD Rental Assistance after initial occupancy has taken place – household was Market, HUD 236 or BMIR, or USDA-RD 515 Basic rent.
  • When Management is processing a new certification/recertification for the household [Annual (HUD & USDA), interim (HUD) or unscheduled (USDA)].
  • With the TC-59hr “Resident Certification Worksheet” when the actual certification/recertification is being completed (everything has been verified).

Please follow these instructions:

  1. The Owner/Owner’s Agent MUST complete the entire form while interviewing the household – every adult member must be interviewed. In households where there are more than two adults, a separate TC-59a must be completed for the third or additional adults. The adults will answer questions as they apply to children.
  1. When completing the interview, DO NOT leave any questions unanswered. Check “yes” or “no” as applicable, enter the “Household Member” number, “Source” names, dates and dollar ($) amounts. ALL questions must be asked and answered! The Interview must be as complete and specific as possible.
  1. On the top of the form enter the property name and unit number. Then check off the relevant type of Assistance Program (more than one choice may apply) and the Type of Request.
  1. List ALL household members starting with the Head, then Co-head/Spouse, next list additional members in order of birth date. If there is an “unborn” child, simply write in “unborn child” where the name should go. Unborn children affect the income limit and bedroom size only at this time. Next enter the relationship to the “head of house”. Relationship codes are found in the instructions to the TC-59hr “Resident Certification Worksheet”, enter these as appropriate to your property. This applies to Foster Adults/Children and Live-in Aides as well – everyone residing in the unit.
  1. Ask the applicant/resident for his/her/their e-mail address and home, cell or work telephone number as relevant. This information will enable Management to contact households in a timely and convenient manner.
  2. The next section is for a “Reasonable Accommodation” request. Property Managers must always follow up on every “Yes” answer with an RS-9 and RS-10 as indicated. NO EXCEPTIONS!
  1. When the Interview has been completed, the Owner/Owner’s Agent will ensure that the “Emergency Contact” section has been updated and that the TC-59a(hr) is signed and dated by all parties: adult household members and the Owner’s Agent. Please ensure that the information listed is specifically related to “each” household member: look under the “Mbr” column for the household member’s number.

NOTE: The signature dates on the TC-59a(hr) Interview should NOT be the same as the signature dates on the 50059 (HUD) or the 3560-8 (USDA). It should be the date the interview is actuallycompleted. This is because the Interview is conducted well in advance of the actual determination of rent and housing eligibility. During a Management Review, Auditors will often check this date. They check Rental Agreement dates; certification dates (50059 or 3560-8) verification dates, etc. to help determine the property’s program compliance.

  1. Next the Owner/Owner’s Agent will determine, from the items checked, which verifications will be needed to complete the certification/recertification - the adult applicant(s)/resident(s) will sign the necessary release clauses on the appropriate verifications that will be mailed/faxed out on their behalf. These verifications are used to determine the household status, income and/or any other relevant circumstances that determine housing eligibility.
  1. The TC-59a(hr) form must be attached to your certification/recertification paperwork when sent to the Occupancy Supervisor for auditing and/or processing. It should be placed UNDER the TC-59hr “Resident Income Certification Worksheet” form along with ALL of the verifications and other relevant household information.
