Form 1 – Confirmation that a school is causing concern or is no longer causing concern

This form must be used by the lead inspector to confirm the team’s judgement that a school falls into one of the categories of schools causing concern or no longer causing concern.

This form must be emailed to and to the relevant ISP immediately after the school has been informed of the decision, and within 24 hours of the end of the inspection. The judgements reported on this form will be included in the data and lists published on the Ofsted website. Any subsequent changes to these judgements must be notified immediately to the same addressees as above.
