Screen shots of mock ups 1 and 2 (contained on page 12 of this document) from the Ebix development environment will satisfy as acceptance criteria.

EbixExchange Work Request Response
A Work Request Response (WRR) is used to determine the level of effort EbixExchange estimates it will require to complete analysis and design on the Work Request. The WRR will contain an Analysis and Design Overview, the EbixExchange Resource Requirements needed to complete the work effort, Analysis and Design Milestones, the Cost to complete the work effort, the Proposed Delivery Date, and any other information that may be necessary.
If EbixExchange determines a WRR is not sufficient to provide the level of detail for this Work Request, a separate Statement of Work will be provided that details the work effort solution.

Project Costs

All Projects will be billed according to the Fixed Cost for delivery of functionality specified within this Work Request, as well as documents specifically referenced within this Work Request, by both EbixExchange (“EbixExchange”), a division of Ebix, Inc. and Massachusetts Mutual Life Insurance Company. New functionality or modification to functionality documented in this Work Request will require collaboration between the client and EbixExchange and may result in additional cost and will be scheduled for a delivery timeline at the discretion of EbixExchange.

The total cost to complete development and testing for work requested hereunder is$1,125 Hourly billing rates used within the calculation of the Fixed Cost will be calculated using the EbixExchange Hourly Rate Schedule within this section.

Payment Terms

The payment will become due and payable as described within the Master Professional Services Agreement (MPSA) between EbixExchange (“EbixExchange”), a division of Ebix, Inc. and MassMutual.

Acceptance & Authorization

Work Requests may be approved by authorized MassMutual representatives. Work Requests under $1,000 maybe approved through WebTrack by noting the proper and full document name along with the version and date of the document in the appropriate WebTrack and comment “approved”.

Work Requests greater than $1,000 and less than $5,000 may be approved by email. To accept this agreement and specified Costs and to authorize EbixExchange to proceed with the development and billing of this request, please attach this document to an email message indicating approval of the WRR and specify Project Costs being approved, and send the email to your EbixExchange Project Manager.

The Acceptance and Authorization of this agreement is effective as of the date you provide your authorization, as specified above.
