Notes to Applicant, Guiders and Parents

Applicant – Please complete this form, if you are under 16 please ensure your parent or guardian signs it. It should be returned to your Unit Leader as soon as possible.

Parent / Guardian – Please ensure the form is completed correctly and sign it to give permission for your daughter to be contacted via email and text.

Guider – Please ensure the form is signed where applicable and forward to your District Commissioner who will input the applicant’s details onto the Girlguiding GO site and then forward it to the Senior Section Advisor (details in County Directory).

All Duke of Edinburgh applicants must be CRB checked if they are over 16 years.

All members (except Rangers only) over 18 years must be CRB checked.
