About Medical Services and Supports for Persons with Disabilities（former；”Kosei-iryo” Medical Services and Supports）

～For a patient will have heart surgery～

Medical Services and Supports for Persons with Disabilities are financial assistance with necessary medical cares for a person with disabled to rehabilitate the owned physical function. However, judgment could decline the application, so that may be unauthorized in some cases

＊To use Medical Services and Supports for Persons with Disabilities, the patient needs to be granted Physical Disability Certificate which involves the dysfunction of the heart. Unless the patient is granted its the involved, the patient needs to apply (apply its re-issuing) the certificate.

【Self pay burden】

Essentially, a self pay burden needs a patient to pay 10% with whole medical expenses (meals and private room are excluded), but a payment for the burden will be set on a limit on each month as an income etc. may change on each month. Also, the pay burden may become ineligible with depended on the grand total of the municipal inhabitant's tax （per income levy）.

☆ The municipal inhabitant’s tax can be defined as an income 2 years ago if applied until June or it can be defined as an income last year if applied after July.

If an application is not confirmed by day before the surgery, Medical Services and Supports for Persons with Disabilities will not be supplied.

★It takes about a month for the result come out after the applying.

【Necessities to apply】

・An Application Form with Medical Services and Supports for Persons with Disabilities-supply（The application will be filled out at the reception in city hall.）

・A Written Opinion and Judgment with Medical Services and Supports for Persons with Disabilities as the Necessity.

・Health Insurance

・Physical Handicapped certificate

＊Unless a patient has; a doctor’s certificate for handicapped, applicant’s photo（4x3 cm; upper body taken）.

・Seal

・A Certificate of Taxation with Municipal Inhabitant’s Tax in Previous Year

（If a patient has resided in the city by January of the applying year, the patient does not need the certificate since the patient has written a letter of consent for a study of the taxation condition at the application filled out. However, a patient who has moved from outside the city needs the certificate.）Notice of Decision etc (For those involved many on medical care expenses at high costs.) As depended on an income classification, (Low income 1, Low income 2) the applicant may need to submit the proof of the pension for handicapped etc to prove the amount of pension that you saved.
