REQUIREMENTS OF APPLICATION
- All applications are to be submitted using the form provided. Forms can be obtained:
A.On-line at Hover over “4-H Youth Development” on the yellow menu bar near the top of the screen. On the drop down menu click on 4-H Resources, then on Awards & Scholarships; scroll down to Scholarships, click on Small Animal Scholarship Application.
B. At the UW-Extension Office, 400 University Drive, Fond du Lac.
C. By contacting one of the following committee members:

Patty Percy at the Ext. Office, 920-929-3170; (Tuesdays & Thursdays)
- Applicant must have been enrolled in one of the small animal projects and exhibit at the Fond du Lac County Fair for at least two (2) years preceding application for scholarship.
- Applicant must participate in the Small Animal Auction as an exhibitor or junior leader.
- Applicant must be a graduating High School Senior to be eligible for consideration.
- Applicant currently enrolled in their first year of advanced education following high school graduation is eligible for consideration.
- All applications must include a complete high school transcript and transcript from anyadvanced education.
- Scholarship proceeds may be used to further education at any accredited four (4) year college, two (2) year college, vocational, or technical school.
- Scholarship will be paid to the member upon the committee receiving confirmation of college enrollment.
- Application and recommendation letter must be postmarked by June 30 of application year.

The FDLCounty Small Animal Scholarship will be Awarded at the
Fond du Lac County Fair during the Small Animal Auction held at 4:00pm on Saturday in the CowPalace.

RECOMMENDATION: Obtain one letter of recommendation from a leader in the youth organization you are a member of. Choose someone unrelated to you who is a general, project, or activity leader, or teacher, but not your parent. This person is to be an adult, unrelated to you, and familiar with your character and personal qualifications for this scholarship. Have the person send the recommendation including signature, title, and date to:
