Flu/Pneumonia Immunization Campaign Materials Available From the Missouri Department of Health and Senior Services Warehouse

Request for Literature Form

This form should be used to order the campaign materials listed below that are available from the Missouri Department of Health and Senior Services Warehouse. The various items available are already listed on the attached Request for Literature form. Simply fill in the quantity of each item needed and fax the form to (573) 751-1574. Please allow 2-3 weeks for orders to be filled.

Literature #105. The flyer is printed with English on one side and Spanish on the other.
Suggestions for use:This flyer is appropriate for use with all audiences to identify those individuals who are at high risk and should be getting a flu immunization. It can be displayed in prominent locations, used as a handout for community awareness displays/activities, or attached to newsletters/bulletins. You might encourage individuals to call the CDC Immunization Hotline provided on the flyer if they have questions regarding flu immunizations.

This brochure is published by Channing Bete Company, and is available in English and Spanish. Literature #81 for English and #82 for Spanish.

Suggestions for use: These materials should be used to encourage people with diabetes to get flu and pneumonia immunizations. The insert and brochures provide information on flu and pneumonia immunizations and make excellent handouts for community awareness displays activities. They can also be attached to newsletters/bulletins or placed at cashier counters and other prominent locations for pickup. The posters should be displayed in prominent locations as reminders to get flu immunizations.

Suggestions for use: This poster is appropriate for use with all audiences. The poster should be displayed in prominent locations as a reminder to get a flu immunization. There is a lot of open space on the poster that can be used to announce local flu shot clinic dates and locations.
