Instruction

a)The Permit should only be issue to Contractors that have already attended contractor orientation and training.

b)Fill out the following sections of the permit: Hot work being done by Flint Project Coordinator Name; Department; Name of person doing hot work; Location; Nature of work; Contractor Supervisor/ Flint employee at the bottom of the permit fill in the; Site name and the dates the permit will be valid.

c)Permit should be issued for one week at a time or less

d)Advise that there must have an extinguisher on site.

e)Once they have been given the permit they will work through the precautions check list as they apply to their project

f)Each day the permit is used the Contractor is required to work through the precautions check list as they apply to their project and date sign on the bottom of the permit.

g)Upon completion of the phase of hot work each day, the permit holder will conduct a 30 minute fire watch.

h)Once the 30 minute period is completed the permit holder will immediately inform security to inform them that the work is completed and they can begin their monitoring period again.

i)Upon completion of the project one copy of the form must be returned to the point of issue so the permit can be closed out. If this doesn’t happen then you should contact the permit holder or Project completion.

j)Make sure that the permit holder has your contact details and upon return of the permit, double check that it has be fully completed and signed off.
