Financial Policy and Agreement

In our office we are proud to be a team whose mission is to deliver the finest dental service available today. We are concerned about your dental care and want to ensure that it is performed in the timeliest manner. In order to assist you with the investment in your dental health, we have outlined our payment options and other office financial information.

Payment Methods: We ask that all accounts be paid at the time services are rendered unless other arrangements have been made with our financial coordinator. Emergency patients must always pay the day of service.

Ø  We accept cash (includes money orders and cashiers checks), personal checks, VISA and Mastercard.

Ø  We are pleased to offer the CareCredit card, the nations leading patient payment program. CareCredit lets you begin your treatment immediately, then pay for it over a period of time with low monthly payments. After filling out a short application, we can process it online and have an answer for you in minutes! Please ask our office staff for a detailed brochure if you are interested, or you can go online to www.carecredit.com for information.

Ø  As a courtesy, we are happy to submit claims to your dental carrier. Due to the changing nature of the insurance industry, it is impossible for us to guarantee any benefit information. We can assist you with estimating what we think your insurance will cover based on the information you provide for us. All fees not covered by your insurance company are your responsibility. To avoid any misunderstandings, you should check with your insurance company or employer, to know what your benefits and exclusions are for your particular plan.

Cancellations and Failed Appointments: Your appointment is time reserved especially for you. If you are unable to keep your appointment, please give us at least 48 hours notice. There will be a cancellation/failure fee of $50.00 per hour scheduled for each appointment cancelled or failed without 48 hours notice.

Finance Charges: If a statement becomes necessary, and is past due, a 1% per month finance charge will be assessed on the unpaid balance. Accounts over 90 days past due are subject to be turned over to a collection agency.
