for Innovation and Collaboration in Delivering the Best Care Possible

New Hampshire Hospice and Palliative Care Organization established this award to honor the wonderful work of Ira Byock and Yvonne Corbeil during their years in New Hampshire. Ira and Yvonne named Dr. Patrick Clary the first recipient of this award, for his contributions to improving end-of-life care in our state by bringing together a network of clinicians and enabling communication, quality improvement, and education about palliative care. Dr. Clary was influential in bringing Ira and Yvonne to New Hampshire, making him a fitting first recipient.
NHHPCO seeks nominations for an individual, team or organization in Northern New England who has demonstrated innovation and collaboration to:
- Improve quality
- Expand access, and/or
- Increase efficiency
in delivering the best end-of-life care possible to New Hampshire residents
The 2016 award went to Kelly Doherty, RN-C, MSN, ARNP-BC, Director of the Palliative Care Program at Cornerstone VNA, who has been instrumental in its success since the inception of the program in September 2014. In her role, she serves as a liaison to local area hospitals and collaborates with assisted living facilities and skilled nursing facilities to ensure that the Palliative Care Program is meeting the needs of the community. Additionally, Kelly serves on the Cornerstone VNA Medical Directors’ team and trains all Cornerstone VNA Home Care nurses in pain and symptom management. Doherty is also a certified POLST trainer (Physician Orders for Life-Sustaining Treatment) and a frequent conference presenter on pain management topics. Julie Reynolds, RN, MS, Chief Executive Officer at Cornerstone VNA states, “Kelly is committed to improving the quality of life for patients and their families, as they strive for wellness or embrace a chronic or life limiting illness with dignity and grace. We are so proud that she is part of the Cornerstone VNA team and that her work was recognized with this most prestigious award.”
The 2017 award will be presented at the NHHPCO Annual Meeting on Friday June 24, 2017.In addition, the recipient’s work will be recognized through state and national press releases, on the NHHPCO website and at the NHHPCO annual conference.
The recipient will be chosen from a list of finalists selected by a committee comprised of the NHHPCO current President, Executive Director, one additional member of the board of directors, and the most recent award recipient. We are honored to also have Ira and Yvonne participate in the selection process.
