Figure A1. Portion of interview guide

1)  People often get and eat food in many different places: home, school, work, church, on the road, on the run, etc. I want you to walk me through your day, yesterday. Please tell me where you got your food, where you ate your food, and everything that you ate? Start with the first thing you ate in the morning and take me right up through the last thing you ate before bedtime

2)  Great. So that’s what you did yesterday. Does what you eat change on different days of the week? (e.g. weekdays and the week-ends?) Does what you eat change for different weeks of the month? How about in different seasons: Winter? Spring? Summer? Fall?

3)  Where would you say you get most of your food? (if “from home”: Where does most of the food from home come from?) Why do you get most of your food there? Does it matter what type of food we are talking about? Do you ever get food from other places?

4)  How do you get to the places where you get food (what method of transportation)? Why do you use this method? What do you think about [method] as the way you get food?

5)  Where would you say you eat most of your food? Why do you eat most of your food here? Do you ever eat food in other places?

6)  Who buys most of the food you eat? Why is that? When does [that person] do the shopping? What does [that person] think about doing the shopping? How does [that person] usually pay for food (prompt: cash, check, credit card, food stamps, WIC voucher, other)?

7)  Who prepares most of your food? What does [that person] think about preparing food? What kinds of foods does [that person] make?

8)  How would you define “healthy eating”? What foods are healthy? What foods are unhealthy? Has a doctor ever recommended a specific diet to you? [If so] What did he/she recommend and why?

9)  What do you think about fruits? (hand participant visual cue for fruits*) Suppose you wanted to help people like yourself eat more Fruits, what would you do?

10)  What do you think about vegetables? (hand participant visual cue for vegetables*) Suppose you wanted to help people like yourself eat more vegetables, what would you do?

11)  What do you think about fast foods? (hand participant visual cue for fast foods*) Suppose you wanted to help people like yourself eat less fast foods, what would you do?

12)  What fruits do you eat? Why these fruits? Are there any fruits you avoid? Why? Are the fruits you eat mostly fresh, frozen, canned, jarred, dried, or pre-prepared (as from a restaurant, take-out, or a microwave dinner)? Why is that?

13)  What vegetables do you eat? Thy these vegetables? Are there any vegetables you avoid? Why? Are the vegetables you eat mostly fresh, frozen, canned, jarred, dried, or pre-prepared (as from a restaurant, take-out, or a microwave dinner)? Why is that?
