ADDRESS CHANGE
How do I change my address with the district?
Employees should log on to Employee Online to change their address. Under Personal Information, click on Home Address. Click Edit, change/update information,and Click Save.
How is Arizona State Retirement notified?
It is important for you to update your personal information (address, name, beneficiary) with AZ State Retirement. Changing your address with the school district does not update your ASRS information. Go to the ASRS website for forms and information.
What happens to my check if it is mailed to the wrong address?
If the check is mailed to the wrong address, the check will be returned to Payroll through the US mail service. It will take ten business days or more before it is returned to Payroll. Payroll does not reissue the check before the US postal system has had 10 days to reroute the check back to us.
NAME CHANGE
How do I change my name?
The District uses the Arizona State Retirement System Change of Address/Name form. The form is located on the CUSD website under Payroll forms. After filling out the form and attaching copies of the necessary documents (see below) to show the change, send it to your Payroll Tech at the District office. The change will be made in our HR/PR system and forwarded to the Arizona State Retirement System to be changed in their database
Do I need a new Social Security card?
Yes, the name cannot be changed in Chandler’s HR/PR database until a new Social Security card is shown reflecting the name change. Attach a copy of the new social security card to the Name Change Form.
If I am certified, do I need to provide a changed certificate?
Yes, if you are certified, your teaching certificate must also be updated with the name change before it can be changed in the HR/PR system. Attach a copy of the new teaching certificate to the Name Change Form.
Is there any other entity that needs that information?
The payroll tech will forward the information to Information Systems and Benefits.
What other forms do I need to fill out?
If there is a change, or you wish to make a change in your filing status for Federal and State Income Tax, go to Employee On Line/Tax Info to make changes. Fill out a new Direct Deposit form to reflect the name change and/or account change.
