Faith Music Fest 5K Fun Run/Walk

Faith Music Fest Family Fun Run/Walk will open the 2017 Faith Music Fest hosted by Open Arms Ministries. All participants will receive a race t-shirt (must register by October 23 to receive a shirt). Refreshments and post party to follow. Online registration and festival information available at faithmusicfest.com
REGISTRATION FEE: $20 per person (kids 10 and under FREE when accompanied by paid adult)
TEAM FEE: $15 per person (10 or more –must register at the same time)

WAIVER AND RELEASE OF LIABILITY
I understand that my, or my minor child, or the minor child in my legal custody (collectively “my” and “I”) participation in the Faith Music Fest Family Fun Run (“Event”) is at my own risk. I understand there are risks associated with the activities of the Event. I agree to indemnify, defend, and hold harmless the Faith Music Fest Family Fun Run organizers, Faith Music Fest, Bethany Lutheran Church d/b/a Open Arms Ministries a/k/a Open Arms Church, Heritage Park, the City of Slidell, St. Tammany Parish, and their respective officers, employees, affiliates, subsidiaries, as well as independent contractors and Event sponsors, from and against any and all claims, liability, judgements, fines, and expenses, including all attorney’s fees and amounts paid in settlement actually and reasonably incurred in connection with any proceeding to which any of Faith Music Fest Family Fun Run organizers, with respect to photographs, motion pictures, video recordings, or any other record of the Event, in which I may be included, and my name or statements made by me, to copyright in its own name or otherwise, to use, reuse, publish, and republish in the same in whole or in part, in conjunction with any printed matter in any and all media now or hereafter known, and for any purpose whatsoever, for illustration, promotion, art, advertising and trade, or any other purpose.
Sign, date, enclose payment, and send to Faith Music Fest Fun Run c/o Bethany Lutheran Church d/b/a Open Arms Ministries, 1340 8th Street, Slidell, LA 70458. Checks are to be made payable to Bethany Lutheran Church. Signature is required for all participants 18 and over. Parent or guardian signature required for participants 18 and under.

Start and Finish: Heritage Park. Relatively flat course takes you through the heart of Old Towne Slidell. Strollers and wheelchairs welcome.
Awards: 1st place medal for male and female ages 16-49; 1st place medal for male and female ages 50+; Team award - (for the most entries of a business or community church).
Packet Pick Up: Open Arms Ministries, 1340 8th Street, Slidell, LA, 70458, Friday, November 10, 5pm-7pm.
Race day pick up: Heritage Park, 7am-8am
