NEW STUDENT REGISTRATION INFORMATION 2014-2015

  • A testing fee of $10.00 is required for new students. Students should have average grades and good conduct.
  • Parents of NEW students must present their child(ren)’s (1) most recent report card (K-8) and test scores (3-8), (2)Original Birth Certificate, (3) Social Security Card, and (4) Original HRS Health Forms (yellow-#3040/blue-#680)
  • All students must have up-to-date shot records. The hepatitis series is required.
  • For admission, each new student must participate in a readiness orientation, and a successful interview between the parent, child, and principal.

Tuition for 2014-15 will be $5,340 per child. (about$534 per month) Scholarships are available.

The cost of books and fees for all students (VPK excluded) will be $360. (Additional $50 charge for iPad insurance)

Registration Fee: A non-refundable registration fee is due at registration.

The registration fee will be refunded only if a child is not accepted.

  1. Step Up for Students (for students entering kindergarten to 5th grade or transferring from public school) –

  1. Voluntary Pre-K4 Program (VPK) - Apply online NOW through the Early Learning Coalition of Duval (208-2044).
  2. The Guardian Catholic Schools Scholarship Fund: This includesindividual or SIBLING (the rate reduction between the first and second and third child in a family) scholarships.

Tuition is paid through a debit to your bank account. You will need to fill out an on-line application through FACTS Management when your student is accepted. Tuition is paid in 10 payments, July through April.

Families not wishing to use F.A.C.T.S Tuition Payment Plan are given the option of the One Payment Plan or the Two Payments Plan made directly to the office. *MONTHLY PAYMENTS ARE NOT MADE DIRECTLY TO THE OFFICE. All families are REQUIRED to have a F.A.C.T.S. agreement for billing.

Every family is required to provide a minimum of 25 hours of volunteer service to the School or give $125 to the Tuition Fund by the second week of May. Hours may be met by participating in our fund-raising activities, e.g., candy sale, Carnival, Raffles, Bake sales, attending School functions, or assisting in the classrooms, office or around the School.

ADMISSION PRIORITIES

  1. First preference is given to supporting members of St. Pius V Church or Church of the Crucifixion.

2.Second preference is given to those families whose children are presently enrolled in St. Pius V School.

  2. All new students.
