Emergency Medicine Physician

Expected Behaviors

General Requirements

It is expected that an Emergency Medicine Physician will:

1. Evaluate the complaints and condition of any patient presenting in the Emergency Department, to provide an appropriate medical screening exam, to provide initial stabilization, and to determine the final disposition of each patient.

2. Actively promote traditional values of medical ethics in which patient welfare is the primary goal of all medical encounters. Actively promote and subscribe to the ACEP published code of ethics for emergency physicians.

3. Maintain a current, active, unrestricted medical or osteopathic license in the state of Idaho.

4. Maintain a current, unrestricted controlled substance pharmacy license with the Idaho State Board of Pharmacy.

5. Maintain a current, unrestricted Drug Enforcement Administration (DEA) registration (schedules II-V).

6. Maintain Specialty Board Certification.

7. a. Have completed an Emergency Medicine residency training program approved by the Accreditation Council for Graduate Medical Education (ACGME), the American Osteopathic Association (AOA), or the Royal College of Physicians and Surgeons of Canada. Initial certification by an American Board of Medical Specialties (ABMS) recognized board (ABEM) or completion of residency within the past 2 years. Current certification by an American Board of Medical Specialties (ABMS) recognized board (ABEM).

OR

b. Have completed an accredited primary care residency training program approved by the Accreditation Council for Graduate Medical Education (ACGME), the American Osteopathic Association (AOA), or the Royal College of Physicians and Surgeons of Canada, or the College of Family Physicians of Canada, in Family Medicine, Pediatrics, Internal Medicine, or General Surgery. Current certification by an American Board of Medical Specialties (ABMS) recognized board. Show a continuous employment history and have spent at least the last 5 consecutive years in full time emergency medicine practice.

8. Have no disciplinary action within the past five years against any medical or osteopathic license; or by any U.S. state, district, or territory; or by any federal agency, including Medicare and Medicaid.

9. Have no record of felony or fraud conviction since graduation from medical school.

10. Have no record of disciplinary action in the National Practitioner Data Bank (NDPDB) within the past 5 years.

11. Maintain active Medical Staff Privileges at Bonner General Hospital.


General Requirements continued

It is expected that an Emergency Medicine Physician will:

12. Maintain appropriate Credentialing requirements of Emergency Department Committee and Medical Staff Credentials Committee at Bonner General Hospital.

13. Maintain and provide documentation of CME to physician personnel file and the Medical Director of Emergency Medicine at Bonner General Hospital.

14. Show completion of courses in ATLS, ACLS and PALS.

15. Maintain adequate knowledge, and practice in compliance with the Medical Staff rules and regulations, as well as Emergency Department and Hospital policies.

Quality of Technical Care

It is expected that an Emergency Medicine Physician will:

1. Maintain Specialty Board Certification.

2. Demonstrate adequate knowledge in Emergency Medicine through attendance at medical CME or home study courses completing 50 hours per year. A minimum of 25 hours per year ACEP Category I credit is expected and should be targeted at areas of clinical weakness.

3. Document all patient encounters at bedside.

4. Document all patient encounters compliant with current HCFA guidelines.

5. Maintain accepted level of standards as set by medical peer review QA chart review process.

6. Adhere to evidence based clinical guidelines where adopted by ED Committee.

7. Demonstrate through a knowledge in Emergency Medicine the practice of evidence based medicine where possible.

Quality of Service

It is expected that an Emergency Medicine Physician will:

1. See all ED patients in a timely manor.

2. Greet patients with a smile and acknowledge patients by name.

3. Sit down when taking a history at bedside.

4. Convey concern for patient’s comfort and privacy.

5. Communicate to patients the anticipated series of events in the ED.

6. Provide clear instructions on treatment and appropriate specific follow up, and discuss what symptoms would warrant a recheck in the ED.


Practice Management Skills

It is expected that an Emergency Medicine Physician will:

1. Start ED shifts at the scheduled time.

2. Demonstrate adequate knowledge of COBRA/EMTALA issues and abides by BGH policy and guidelines for compliance with appropriate MSE and patient transfers.

3. Communicate in a professional and courteous manor with the consultant MD providing the concise necessary clinical situation and reason for referral.

4. Manage ED patient flow and avoid prolonged processing delays.

Resource Utilization/Economic Efficiency

It is expected that an Emergency Medicine Physician will:

1. Participate regularly in departmental committees developing practice guidelines.

2. Participate regularly in monthly discussions of aggregate departmental utilization data when available.

3. Review own utilization data when available at least quarterly and seeks additional information as appropriate to understand personal use patterns.

Peer/Co-worker/Team Relations

It is expected that an Emergency Medicine Physician will:

1. Behave in a professional and cooperative manor with ED staff. Maintain good rapport and demonstrate constructive problem solving with teammates both medical and non-medical.

2. Regularly seek input from non-physician co-workers.

3. Resolve conflicts with peers and co-workers in a private setting.

4. Transfer care of low medical-legal risk patients at shift change with appropriate clinical information to include clinical history, diagnosis or differential diagnosis, abnormal test results, additional testing or treatment needed and a disposition plan.

5. See all patients with potential life-threatening complaints up to the scheduled shift change. Not cause undue prolonged patient wait times by “saving patients” that present with non-life threatening complaints more than 20 minutes prior to the shift change for the next physician. Facilitate patient care and decrease processing time by pre-ordering simple tests on patients who present near shift change with a simple problem.

6. Makes positive contributions to group discussions.


Contribution to Organization and Community

It is expected that an Emergency Medicine Physician will:

1. Represent the ED well in the media and the community.

2. Be a member of two or chair of one hospital medical staff committee.

3. Attend at least 80% of the ED Committee meetings per year. Part-time physicians will be encouraged to attend ED Committee meetings and will be expected to attend at least a percentage of the time directly proportional to the amount of time scheduled. (e.g. EDMD scheduled for 96 hours per month would be expected to attend approximately 50% of the ED Committed meetings 96/168=57%)

4. Attend at least 80% of the Medical Staff meetings per year.

5. Attend at least 80% of the corporate business meetings per year. Part-time physicians will not be required to attend corporate meetings.

6. Gives appropriate advance notice for vacation, CME, and other time off and does not cause undue adverse scheduling events for fellow EM Physicians.

7. Participate when requested, in one TQM project related to department activities every 2 years.

8. Participate in a leadership role in a medical association or organization. (ACEP, ID/ACEP, IMA etc.)

or

Participate in a leadership role in two of the following areas of community service per year:

a. Event Medicine off-site medical care for series of yearly scheduled events. (i.e. Festival at Sandpoint, Longbridge Swim, Lake to Forest Triathlon etc.)

b. EMS Medical director for on agency/provider or district.

c. Courtesy Physician for Schweitzer Ski patrol program.

d. Continuing Education Director for ED staff. (CME, Hot topics, ACLS, ATLS, PALS etc.)

9. Participate in the WRITE Program as a Preceptor for medical students. Maintain appropriate credentials through the University Of Washington School of Medicine as an auxiliary faculty member through the Department of Medicine.

Full Time Position Requirements

It is expected that an Emergency Medicine Physician will:

1. Be scheduled for a minimum of 156 hours per month. Scheduling for more than 195 hours per month will be strongly discouraged.

2. Be scheduled for a minimum of 100 hours per month if concurrent role as Medical Director.

3. Be able to work a combination of 12 or 24 hour shifts if requested.

4. Submit requests for specific days off or blocks of time for Vacation or CME to the medical director a minimum of 6 weeks prior to the start of the scheduled month. Requests to be honored in order of time request received or posted.

5. Be scheduled for a equitable amount of holiday shifts distributed evenly with peers.

6. Be scheduled for a equitable amount of night shifts distributed evenly with peers. Scheduling for more than three 12 hour night shifts in a row will be strongly discouraged. Scheduling for more than four 12 hour day shifts in a row will also be strongly discouraged. Scheduling for 24 hour shifts every other day will also be strongly discouraged.

7. Take individual responsibility for all schedule changes made after the final schedule is completed (6 weeks prior to start of month), and will arrange shift switches and appropriate ED coverage for the dates in question.

8. Not cause undue adverse scheduling events with peer physicians by requesting frequent large blocks of time off.

9. Give the medical director 2 months advance notice and assist the medical director in obtaining adequate ED coverage during any absence that exceeds 2 weeks.

10. Give the medical director 3 months advance notice of resignation of position.

11. Scheduling of shifts at LPOEM contract sites other than the primary coverage hospital will be distributed evenly with fulltime peers. Any additional shifts or hours will be at the sole discretion of the individual physician.

Part Time Position Requirements

It is expected that an Emergency Medicine Physician will:

1. Submit dates of availability to the medical director a minimum of 6 weeks prior to the start of the scheduled month.

2. Take individual responsibility for all schedule changes made after the final schedule is completed (6 weeks prior to start of month), and will arrange shift switches and appropriate ED coverage for the dates in question.

3. Be scheduled for a maximum of 120 hours shifts per month.

4. Be able to work combination of 12 or 24 hour shifts if requested. Scheduling for more than three 12 hour night shifts in a row will be strongly discouraged. Scheduling for more than four 12 hour day shifts in a row will also be strongly discouraged. Scheduling for 24 hour shifts every other day will also be strongly discouraged.

5. Give the medical director 3 months advance notice of resignation of position.
