Make a significant contribution to the development and implementation of corporate and / or service strategies

Job context
To provide a range of social care services to older people and people with a physical disability or impairment. To deliver services to people who need a rapid response to a crisis, need help to maintain their independence where they have complex needs, and to promote preventative services which help people remain well and independent. To ensure that safeguarding procedures are in place to support vulnerable people. To ensure that the Council performs its statutory duty in relation to the provision of Adult Social Care services.
The delivery of adult care is a high profile public service provided by the Council in partnership with the local NHS though contractual and partnership agreement, the Voluntary & Community sector and user-led organisations.
Adult care consists of the following teams:

- MASH
- Reablement
- Ongoing Support
- Hospitals & Integration
Job purpose
Specific duties and responsibilities of the head of service hospitals and integration include:
- Lead the adult care hospital and integration team to manage its performance, to allocate resources, manage risks and provide leadership and inspiration to staff and managers. These activities to be implemented in line with current legislation e.g. The Care Act 2014
- Lead adult care element of the intermediate care team ensuring the effect use of the bed base and timely discharge to onward location
- Lead and be the expert for CHC within the SLT and negotiate with health partners when determining responsibility within relevant legislation and guidance.
- Ensure the highest levels of public satisfaction for the service, to continue to transform the service to meet the aspirations of customers and in accordance with the requirements of the law and external regulation
- Lead role in the Senior Leadership Team (SLT) of the service and to lead and deliver a performance culture to achieve the desired outcomes across the service for customers and external regulators.
- Participate in setting budgets and to manage the deployment of all the resources for operational teams and to take action so that resources are used effectively, efficiently, economically and in a consistent manner, so that service expenditure remains within budget and meets the targets set out within the service and corporate plans.
- Understand performance management information and to ensure that all managers and staff within the service are clear about their role in delivering performance outcomes. To interpret and analyse performance management date and to take action to improve continuously performance within adult care.
- Lead and embed an asset based approach throughout the remit of their role
- Ensure that the service responds appropriately and efficiently to individuals within the acute and community hospitals to facilitate to ensure they are receiving the right level of support on discharge and maximise their independence
- Work in collaboration with acute and community hospitals to deliver zero delayed transfers of care
- Working with partner agencies within health to deliver joined up services that mean customers receive seamless delivery of services. Maximise opportunities for joint working and pushing forward the integration agenda.
- Draft as required for the Director responses to complaints and Members enquiries, policy documents, papers and committee reports.
- Work in collaboration with across service areas within the Council to deliver key priorities identified in the business plan
- Engage with customers, carers and the wider community to inform continuous improvement and delivery of high quality adult care services

Person specification
Specific qualifications, knowledge, and skills required for this role:
- Educated to post graduate degree level or ability to demonstrate equivalent skills
- A professional qualification relevant to Adult Social Care with evidence of learning and development and extensive management experience.
- Substantial experience of working social care with a sound up-to-date knowledge of relevant legislation and guidance in relation to social care law and regulation
- Extensive experience of managing sensitive and complex cases and an ability to develop effective partnerships with external providers
- Knowledge and understanding of commissioning and its role in Adult Social Care Operations.
- Significant experience working on large projects and managing change
- Substantial experience of leading and delivering successful cultural and organisational change programmes both within the function and with impact across the council
- Significant experience of leading effective partnership working across health, social care and the 3rd sector
