VOLUNTEER, CHAPERONE AND DRIVER AGREEMENTS

Active engagement in the classroom and fieldwork are essential components of Evergreen’s learning design. Parent volunteers, chaperones, and drivers are critical to the full and safe implementation of these components. To insure that drivers, volunteers and chaperones fully understand their obligations, we ask that you read these agreements and keep them in your glove compartment for future reference.

Drivers’ Agreements

  • I have submitted a copy of my valid N.C. driver’s license.
  • My car is in good repair (with a current state inspection).
  • I will carry a cell phone so that I can contact the school or teacher in the event of an emergency.
  • I understand the child restraint laws and will make sure that students abide by them when riding in my vehicle. (the current law stops short of what is recommended for child safety. Evergreen supports practices for best protection and this includes keeping children in properly fitted booster seats with lap and shoulder belts until the child is big enough for just the lap and shoulder belt. Add-on shoulder belt adjusters are not recommended and shoulder belts should never be tucked under the arm or behind the back. Children under 80 pounds should sit in the rear seats of the car.   • I understand that caravanning as a means of travel (whereby many vehicles must follow a single or multiple leader vehicle(s) to arrive at a destination) is unsafe, as drivers who attempt to keep up with the leading vehicle through stoplights and turns may put themselves and their passengers at risk.
  • I will make sure that I have clear and concise written directions to each site, as well as a contact phone number of the destination point if applicable and a contact phone number for the Evergreen Leader in charge.
  • I will not deviate from the route designated by the written directions or make unplanned stops for personal business (including snacks, stops at home, errands, etc.)
  • I will not drink alcohol, use illegal drugs, or take any medications that may hinder my driving while chaperoning an ECCS field trip
  • I will adhere to the schedule designated by the written itinerary, taking care to arrive at the field site and back at ECCS when instructed to do so
  • I will contact the teacher and the school office in the event of a medical emergency, car breakdown, or necessary delay/change in the schedule or route
  • If I have committed to drive on a field trip and cannot do so, I will contact the teacher as soon as I know so s/he can make other arrangements.
  • I understand that if I fail to uphold these agreements, I may be banned from driving on ECCS field trips for at least the remainder of the school year.

Volunteers’ & Chaperones’ Agreements

  • I understand the importance of confidentiality of student information.
  • I understand that the lead teacher is the class/trip leader and I will comply with the rules and procedures communicated by teachers regarding behavior and consequences, for both chaperones and students, including my own child.
  • If I have committed to chaperone on a field trip and cannot do so, I will contact the teacher as soon as I know so s/he can make other arrangements.
  • I will not drink alcohol, use illegal drugs, or take any medications that may hinder my ability to volunteer or chaperone while at school or on an ECCS field trip
  • I understand that if I fail to uphold these agreements, I may be banned from volunteering or chaperoning at ECCS for at least the remainder of the school year.

I plan to volunteer occasionally in the classroom or office, at events, or on campus.

I plan to volunteer consistently or regularly in the classroom and/or may have unsupervised contact with students and have submitted my information for a criminal history and motor vehicle background check.

OR

I do not plan to volunteer consistently or regularly and/or will not have unsupervised contact with students.

I plan to volunteer as a chaperone on fieldwork and have submitted my information for a criminal history and motor vehicle background check.

OR

I do not plan to chaperone for fieldwork.

I plan to volunteer as a driver for ECCS sponsored trips and have submitted my information for a criminal history and motor vehicle background check. I certify that my vehicle is currently registered, in good repair, my license is current and has not been suspended or revoked, that I have not had any moving violations or DWI arrests in the past five years, and that I maintain at least the minimum amount of liability insurance required by the state of North Carolina. I have submitted a copy of my valid NC Driver’s License to the school. I also understand and agree to comply with Evergreen’s Chaperone/Driver Obligations and Agreements, which include having children in adequate restraint for the size and age of the child.

OR

I do not plan to do any driving for ECCS sponsored trips.
