EP/EH instructions for registration and updating registration for the NLR. These will be the instructions to follow whether this is the first time to register or have the need to update information.

- After reviewing the criteria for eligible professional or hospital click continue
- Step 1 - is the acknowledgment statement. After checking the acknowledgment box, click continue
- Step 2 - If you are updating registration information use your NPPES ID and password to access. If this is the first time, you will need to follow the instructions and create a login (screen shot instructions available at the below link to CMS).
- Step 3 - You will arrive at the “welcome screen” where you will have options to register, attest, or check status. Click on the registration tab
- Step 4 - You will arrive at the registration page and follow the instructions. On this page, you can register for the first time, modify and existing registration, cancel participation, reactivate a previously canceled registration, or resubmit a registration.
- If you have an existing registration it will display at the bottom of the registration page.
- Step 5 – You will be required to fill out the reason for this registration. 3 topics must be completed: EHR incentive program, personal information, business address and phone number.
- Step 6 – Incentive program questionnaire will need to be verified or filled out – click save and continue
- Step 7 – Personal information will need to be verified, modified, or filled out, click apply and save and continue, follow the prompts if you will be attesting as a group. If there are any errors with your personal information, they NLR system will alert you and allow revisions.
- Step 8 – Review the business address and phone number for where the incentive payment will be sent. Red asterisks denote required fields. Save and continue.
- Step 9 – proceed with submission at the bottom of the page if all of the topics show “completed”
- Step 10 – Verify registration give you a quick overview if all the information you just filled out
- Step 11 – You will accept, agree, and submit after reading the registration disclaimer. You have the option to agree or disagree.
- Step 12 – Submission receipt. You will receive immediate confirmation that the submission was successful. You will also be given registration tracking information. It is a very good idea to print this page as you will need this registration ID when you receive clearance to proceed to the SLR.
- Step 13 – you can return back to this site if you would like to check the status of the registration at any time. You will see this by clicking on the “status” tab.
After submitting your registration and printing off the confirmation page, CMS is notified of the new or updated registration.

Note: You will receive a notice in your email from CMS letting you know that you are clear to proceed to your State’s SLR for attestation for Medicaid. . This is where you will need the number from the confirmation you printed so be sure to keep that handy.
