____The student has earned a grade of B or better in the most recent course that is the same content area of the course for which the student is attempting the EOC.

____ By signing below, I, the Parent/Guardian, give my child permission to take the EOC Test-Out option. I have checked the boxes below to indicate I understand the following:

  I am responsible for the $50 test fee, which is due on March 9, 2018. I understand that the fee is only refundable if my child earns the achievement level of Distinguished on the EOC (Grade Score Conversion of 92). (Attach test fee to registration form.)

  Once the test has been taken and the credit from this test-out option has been posted to the official transcript (given the student scores in the Distinguished achievement level), I understand that my student has completed the course and may not register for this course in the future. I also understand that neither my student nor I may decline a Distinguished score.

  If my child is in high school, the Distinguished score will be recorded on the student’s transcript and will be calculated in the student’s GPA. If my child is in middle school, the Distinguished score will be recorded on the student’s transcript but will not be calculated into the student’s high school GPA.

  Honors, Accelerated, IB, and AP designations do NOT apply to EOC Test-Out options.

  I understand that the Test-Out Option may impact my child’s NCAA eligibility.

  This form is due to the school administration by Monday, March 9, 2018.

  The EOC Test-Out administration will be Thursday, March 22, 2018.
