READ CAREFULLY AND PLEASE FOLLOW DIRECTIONS.

Please enter the rankings from the Evaluation Form. Use the final score, not the mid-point score. If a teacher marks 2 choices, choose the LOWER score unless you feel it should be the higher grade.

Entering Student Teaching Evaluation Data

1st placement supervisor should enter the evaluations from the first experience.

2nd placement supervisor should enter the evaluations from the second experience.

Early Childhood/Childhood & Fredonia-Hamburg will have a third experience.

TESOL placement should enter the evaluation for the placement.

You may start submitting the evaluation data when you come to the end of the assignment.

  2. Above the picture find the 4th category “Offices”. Click the drop down and the 4th office isOffice of Field Experiences. Click on the words.
  3. You are now on the Office of Field Experiences Website. Under LINKS on the right hand side select College Supervisors. Select the blue lettered Student Teaching Evaluation Form.
  • Type the candidate’s last name and first name.
  • Use the arrow as a pull down and select the correct program (certification). Check the Assignment Form in the Student’s folder to verify the certification area.
  • Select the placement period
  • Select the grade level(s). If multiple, select all that apply.
  • Select Content of the Placement. If multiple, select all that apply.
  • Input the School District
  • Input the School Building

Then each section of the evaluation form appears. Click the appropriate ranking score. After #42 there is a section entitled, “Additional Program Specific Evaluation Items.” Make the appropriate selection and click continue. “Other” is for certifications that do not have additional statements to evaluate.

Entering Disposition Data

Last Placement Supervisor

At the end of the student teaching evaluation you will find a disposition form. The teacher candidate’s last supervisor will submit this form so that each candidate will have one form completed for the semester. For each statement select the appropriate score. Anyone below standard should have an Intervention done during the placement so briefly describe why in the box.
