Amy’s School of Dance Policies 2015-16

Tuition

Tuition payments are due the first week of each month. If tuition is not received by the 10th of each month, a $10 late fee will be added to your account. The late fee will carry over from month to month until paid.

Tuition is a seasonal fee broken down into nine payments. It is not based on the amount of classes per month and holiday closings are figured into this amount.

All returned checks will be charged a $25 fee. Accounts showing two returned checks will be asked to pay remaining payments in cash.

If you have problems paying your account, please talk with the directors to discuss alternative payment arrangements. We will try our best to accommodate a request for alternative payment plans. If you don’t contact us, it will be assumed your account is delinquent.

Acceptable forms of payments are cash, check, debit/credit card.

Attire

Leotard and tights of any color are required for all classes – NO JEANS. Hair must be pulled away from face. We will not compromise on this rule as it is for each child's safety. Boys wear a tight shirt and gym shorts. *Hip hop students may wear loose fitting pants.

Shoe Requirement

TAP CLASSES - Tan tap shoe

BATON, JAZZ & HIP HOP- Tan jazz shoes *Hip Hop may wear a clean sneaker that has not been worn outdoors

BALLET - Pink ballet shoes

LYRICAL/MODERN – foot undeez

TUMBLING - Barefoot

CLOGGING - Black clogging shoes

Attendance

All students must attempt to attend all regularly scheduled classes. Good attendance is imperative, as absences and tardiness can hold back an entire class. No refunds or make ups will be given for missed classes.

Snow and extreme emergency cancellations will be based upon the Winterset schools. If school is dismissed early, there will be no dance class. If school is cancelled, dance class is cancelled.

Parents and children under 5 years old must remain in the lobby during the entire class.

If your child is not picked up on time, you may be subject to a fee up to $30 per half hour. Young students should never be left unattended in the lobby.

Communications

Communication will be sent to the email addressprovided at registration as well as the website.Please read these communications and be sure to check the website for updates!!! This is the best form of communication we have for relaying important information. It is the parents and students responsibility to be aware of what is happening at the studio.

Insurance

Amy's School of Dance does not carry medical insurance forits students. It is required that all students be covered by their own family insurance policies. In the event of an injury, it is understood that the student's own policy is the only source of reimbursement.

Costumes

There will be a flat fee per costume. Each child size costume is $60, Adult size is $70. These will be broken up into 2 payments (October & November), which will be added to your monthly tuition.

Costumes are not custom made and students are responsible for all alterations. If the studio does not feel your costume needs to be exchanged for size and you request that it is, you will be responsible for all exchange and shipping fees.

The recital fee is due with your APRIL tuition.

Drop/Change Policy

If you choose to drop a class or discontinue classes with Amy's School of Dance, youmustfill out a drop/change form with us by the 20th of the month. Informing the teacher or failure to report to class does not constitute a drop. If a class is dropped after the 20th of the month the account will be charged for the following month’s tuitionand any applicable fees; accounts will continue to be charged until a drop/change form is completed or written communication is provided to the directors.

In the event a student needs to change classes please complete a change form with the instructor by the 20th of the month. Once the change request is processed the account will be updated and any difference in monthly payment will be applied to the account unless otherwise instructed by the requestor.

Miscellaneous

• Pictures will be taken throughout the year and could possibly be used in promotions for the studio and/or newspaper articles about the children's successes.

• We are NOT responsible for lost or stolen items.

• No food or gum allowed in any of the classrooms. You may bring in WATER ONLY to lessons.

• We reserve the right to refuse service.

No Refunds

We limit our class size and each child has reserved a spot that could have gone to another student.

As a participant in any program or class of Amy’s School of Dance I recognize and acknowledge there are certain risks of physical injury and I agree to assume the full risk of injuries, damages connected with or associated with such program or class.

I agree to waive and relinquish all claims, or my minor child, may have as a result of participating in any program or class against Amy’s School of Dance and its officers, agents, employees and independent contractors.

I further agree to indemnify and hold harmless and defend Amy’s School of Dance and its officers, agents, employees and independent contractors from my claims resulting from injuries, damages and losses sustained by me or my minor child that arise out of, in connection with or in any way associated with the activities of this program.
