Employee and Student Health

Dear First Year Medical Student:

Student Health Services welcomes you to the Loyola University Health System. In order to ensure your compliance with Loyola Employee and Student Health and safety requirements, please complete the following by June 30, 2017.

You will need to enroll in Castlebranch (https://portal.castlebranch.com/LG77), a secure online medical document manager, and click “place order”. This enrollment is mandatory. This will take you to a page dedicated to SSOM students to register for a file to be created to upload information to the site. There will be a $35 one-time charge for this procedure. Some of you may already have an account with Castlebranch from your undergraduate school. For students that already have an account with Castlebranch, you will place your order the same way but you have to use the same email address you used as an undergrad to prevent creating two accounts. Once in the account you can move your old information into your new tracker from the Document Center.

You will need to obtain quantitative IgG AB (antibody) immune titer lab results along with other requirements listed below and upload them to Castlebranch by June 30, 2017 (Note: You may upload previous titer results).

1.  Download the Loyola Health History and Respirator fit testing questionnaires available on the Castlebranch site. Complete the forms and upload them. You do not need a signature from your physician. It will appear as being rejected until it is reviewed by Student Health.

2.  Upload Immune quantitative IgG AB (antibody) titer lab results (proof of vaccination is NOT acceptable) for:

a.  Measles (rubeola) IgG antibody

b.  Mumps IgG antibody

c.  Rubella IgG antibody

d.  Varicella IgG antibody

e.  Hepatitis B Surface antibody

If you are NOT immune to any of the above viruses or the result is equivocal, you will need to have a booster vaccine, then repeat the titer in 4 weeks.

3.  Quantiferon Gold blood test (Tuberculosis blood test) in the past 3 months with a negative result. If you have a positive Quantiferon Gold result, you will need proof that you are negative for active disease by to uploading results of a negative Chest PA X-Ray within the past year. You will also need to discuss this positive result with your physician to obtain treatment for latent TB. A mantoux TB skin test is NOT acceptable.

4.  The only vaccine that you need to upload is proof of TDaP received after June 2012. TD is not acceptable.

5.  Flu vaccines will be provided to you in October during flu season at no cost.

6.  Do NOT upload a CPR card until you complete your onsite AHA BLS during orientation week.

7.  You will need your immunization records (ie. MMR, varicella, DTaP, Hepatitis B, Polio vaccines) for your 3rd year medical school rotations. You can upload the Hep B vaccinations with the immune Hep B titer.
