Community Pathways Project Navigator / Assertive Engagement Case Worker

Full- time Position(40 hrs./week) Non-exempt

Reports to: Family Strengthening Programs Program Manager & Housing Services and SNAP Outreach Coordinator

POSITION SUMMARY:

This position is a combination of two 20 hour positions, the Community Pathways Project Navigator and the Assertive Engagement Case Worker.

The Community Pathways Project is a collaborative of social services agencies lead by Project Access Now. The work of the collaborative is to promote the well-being of families whose members have been affected by gangs, incarceration and poverty. The program addresses barriers, provides resources and case management to promote health, stability and economic mobility.

The Assertive Engagement Case Worker will recognize that how assistance is offered, is far more important than the assistance provided; therefore they must approach each client with empathy, active listening skills and the ability to be an advocate for the client. The primary function of this position is to assist clients, who are dealing with possible eviction, to remain stably housed through a crisis needs assessment and interview process. With the understanding that the client knows best what they need to do to solve their current situation, this position offers resources and assistance as needed.

COMMUNITY PATHWAYS PROJECT (20 HOURS PER WEEK)

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Recruit program participants from community outreach and internal referral efforts.
  • Screen potential clients for eligibility.
  • Engage clients using Assertive Engagement, Trauma informed and strengths based models.
  • Meet regularly with clients to identify, initiate and monitor pathway progress.
  • Organize and maintain records and documentation per agency and contractual requirements in a confidential manner.
  • Provide Navigation services to at least 10 families living in Multnomah County.
  • Connect clients to community resources to complete pathways.
  • Enter client data in CLARA regularly to track pathways for clients.
  • Enter client data and case notes in Salesforce
  • Attend monthly meetings with the Community Pathways Project Navigators team and other meetings as required by the collaborative.
  • Adherence to El Programa Hispano Católicopolicies and procedures.
  • Perform other duties as assigned by EPHC supervisor.

ASSERTIVE ENGAGEMENT CASE WORKER (20 HOURS PER WEEK)

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Verify eligibility based on eviction and homelessness risks as well as income; follow funder’s guidelines to assist eligible clients gain access to rent assistance and the Multnomah Stabilization Initiative (MSI) program.
  • Provide Assertive Engagement case management to clients engaged in the Multnomah Stabilization Initiative (MSI) program.
  • Active participation in the client selection process as outlined in the MSI contract.
  • Verify eligibility and assist eligible clients gain access to emergency energy assistance and or any other service as needed to promote family stabilization.
  • Find appropriate workshops and classes for clients.
  • Be able to do house inspections.
  • Promote family budgeting and self-sufficiency with all clients, in case plans and in one-on-one meetings.
  • Teach classes on budgeting and money management.
  • Assist with tracking of agency resources available for rent assistance.
  • Attend County meetings for rent assistance coordinators (as requested by supervisor).
  • Enter data into Service Point
  • Maintain confidential client files in compliance with El Programa Hispano Católico, Multnomah County and Home Forward standards.
  • Adherence to El Programa Hispano Católico policies and procedures.
  • Perform other duties as assigned by EPHC supervisor.

QUALIFICATIONS AND REQUIREMENTS:

  • Must be bilingual (Spanish/English) and bi-culturally competent.
  • Ability to communicate concisely and effectively, both verbally and in writing in Spanish and English.
  • Bachelor’s degree or equivalent experience required.
  • Experience and knowledge working with a broad range of issues affecting Latinos/Communities of Color in the community.
  • Experience with case management.
  • Experience with community networking and organizing.
  • Ability to build relationships among partner agencies and the Latino community to promote services.
  • Demonstrated experience in facilitating and delivering trainings and presentations.
  • Intermediate proficiency in all programs of Microsoft Office; experience with data entry.
  • Demonstrate judgment and discretion in dealing with confidential matters.
  • Commitment to EPHC/Catholic Charities mission to work in partnership with vulnerable populations to achieve lasting solutions to poverty and injustice.
  • Must be able to organize and prioritize work, be proactive, take initiative, follow through, and simultaneously manage multiple priorities to ensure goals are met in a timely manner. High attention to detail required.
  • Willingness to learn new skills and take on new responsibilities.
  • Strong analytical and strategic problem-solving skills.
  • Ability to work well in team setting, as well as independently; be flexible and adapt well to different dynamics in a fast paced work environment.
  • Ability to support organizational and program specific mission and goals.
  • Ability to maintain a flexible schedule as needed. Driving is required; some travel in the Gresham and Portland area. Valid driver’s license and automobile insurance at agency-required levels (100/300/100).
  • Ability to lift 40 pounds.
  • Satisfactory results from civil, criminal and motor vehicle background check required.
