Name of Primary Assessment:Assessment Portfolio
Directions to the Student:
1). Introduction to portfolio must include: description of teaching setting, types of data collection methods typically used, ideas for making time for authentic assessment and portfolio reflections. A rationale and implementation strategy must be included for both authentic assessments and standardized assessments in the educator’s classroom (each teacher must implement, test and evaluate at least 3 new authentic assessment measurements and at least 3 new standardized assessment measurements).
2). Applied Assessment Results/Analysis: Begin each section of the “applied assessment” section by identifying additional domains/skills that can be assessed using the method (i.e. checklist). Include the assessment and the data collected. Provide at least 3 additional resources and/or forms you might provide for parents/guardians and other professionals that would assist in growth for the child.
3). Assessment Reviews: Several standardized assessments will be reviewed and included in the portfolio.
